I found that Mark Fluharty had put new identification stickers on the markers and the site was marked with paint for the phone lines. I spoke with Mark Fluharty and he said that they had to wait 48 hours for the locate service to come out. The south marker was still venting gas. Mark said that once the area was marked they would dig up the line and run plastic under the road. I told Mark that they will also need to address the vegetation kill on the bank, and I had also notice a dead tree directly behind the marker. |
0620891150 | 2017-05-15 | Production Wells | Violations Noted | I got a call from Mark Pattison with Columbia Gas to report a gas leak from roadside line markers at 46718 County Rd 405. He said that he was called out for a gas odor by the homeowner. Mike said that there was no gas found around the home, and Columbia Gas does not supply that area. I met him at the site and found that there was a visible gas flow from the marker on the south side of the road. I also found that the soil on the roadside embankment was dark and bare of vegetation by the marker on the north side of the road. Mike showed 100% gas in the soil with his detector. The identification on the markers was for Eastern States Oil & Gas and the number was disconnected. I called Northwood Energy pumper Mark Fluharty because they have many wells in the area. Mark confirmed that it was their gathering line. He said that he would have someone out that evening and he would call locate services to mark the area. I went to the home to let the homeowner know that Northwood Energy would be out to repair the line, and to give her my contact information. |

There was freshly excavated soil on both sides of the road. I called Mark Fluharty and he said that Knox Energy sent a contractor out to do the work. He tapped into Northwoods line on the south side of the road and ran plastic up to a house gas meter for the neighboring property. Mark said that he would meet me at the site next week to discuss the remediation of the area that was impacted by natural gas killing the vegetation and the small tree. |

The tank battery consisted of one 100 barrel steel tank and one horizontal separator. There was an earthen containment dike in place around the production equipment.
