0193784072 2015-07-21 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with tubing and rods. The pumping unit had been removed. The well was shut in and there was pressure on the gauge. It was leaking crude oil onto the ground and into a hole next to the wellhead. There was an identification sign at the wellhead. The production reports show 0 bbl. oil and 0 mcf gas from this well since 2010. Remove the crude oil contamination from around the wellhead within 30 days. The well shall be plugged within 180 days.
0518103437 2017-10-31 Production Wells No Violations On the day of my inspection I went back out to the well to investigate the line off of the surface casing. I tried to call the McDonalds, but I was unsuccessful. I called Clayton King, because he claims to own the well, even though the form 7 has never been submitted for the transfer. He claimed that the line has been on there since he got the well, and Greg McDonald was using it to wash fire clay down the annulus. I told Clayton that it must be removed immediately, that this well is not approved for annular disposal. Clayton maintained that he was innocent of pumping water down the backside. He said that he would get the line removed today.

I called Clayton King. He claimed that he would get the pump jack set and the oil cleaned up over the weekend. I also spoke with Greg McDonald. The well has been sold to Clayton King, but the transfer has not been finalized. No form 7 has ben submitted. Mr. McDonald is aware of the violations and the date by which they must be addressed.
0937765383 2016-01-28 Production Wells No Violations On the day of my inspection I found that the pumping unit was set with a combustion engine. All of the belts and weights were in place and the bridle was connected to the polished rod. The unit was in operation during the inspection. I could feel fluid surging through the 2 steel flow line as the well was pumped. There was 400 pounds of gas pressure on the casing. The oily soil around the wellhead head been removed. This violation is resolved. The well has been made capable of production and the contamination has been addressed.

He also agreed that he had purchased the well and he was waiting to apply for a transfer because the well has an active violation.

The oil-soaked soil around the wellhead had been covered with leaves, but not cleaned up. The wellhead was still equipped with rods and tubing. The pumping unit had not been installed.

I got no answer so I left a message and Mr. McDonald called me back. He claimed that he has also been pushing Clayton to get the well back in production. I happened to see Clayton outside when I drove by one of his tank batteries. I stopped to talk with him and he claimed that he was on his way to the well to get the pump jack set and to clean up around the wellhead.

The belts were not on and the polished rod was not connected to the bridle. I called Clayton King and he claimed that his goal was to have the well pumping by the end of the day.
1509458941 2017-10-30 Production Wells No Violations On the day of my inspection I found that the well was equipped with a pumping unit that had no means of propulsion. The flow lines were connected at the wellhead and the valve to the main line was open. The house gas valve was closed. I noticed a suspicious line coming from the ground that appeared to be connected to the surface casing. The line was equipped with a newer stainless steel male cam-lock fitting. I didnt have time to dig the line up and investigate further. I will return tomorrow.
1509730314 2017-11-03 Production Wells No Violations On the day of my inspection I found that part of the line connected to the surface casing nipple was still in place and buried. The main line with the cam-lock fitting had been removed. The remaining line was fitted with a valve and capped with a solid plug. Clayton King called me while I was at the well and left a message saying that he would come back this weekend to completely remove the line and screw the plug directly into the surface casing nipple.
1558448362 2019-05-21 Production Wells No Violations On the day of my inspection I found that the pumping unit was overgrown with thick vegetation, and there was no mean of drive to pump the unit. The flow lines were connected at the wellhead. The tubing valve was open to the tanks, and the casing valve was open to a domestic gas line and new regulator. The gauge on the casing showed 600 pounds. There were no leaks around the wellhead. This well shares production tanks with permit 4662 and there was no identification on the pumping unit. Identification was posted at the tanks and I found no leaks or spills. I called Clayton King to ask about the well. He said that it makes domestic gas for his farm and he was in the process of reconnecting the gas lines so he could get the gas sold down. He said that he had a motor for the unit, and he would get the vegetation cleaned up and get ID posted at the unit. Finally, Clayton said that he was working with the State to get his 2018 production reports up to date.

I met with landowner Alan Kukla at the well. He claimed that he was assisting Clayton King with the clean up. I let him know that the well needs to be made capable of production and the oil needs cleaned up.
