0032054354 2014-01-21 Administrative Inspection No Violations FOLLOW UP TO CA #386042156: I received a phone call from Eric Brown, who reported that Clayton King of Royalty Enterprises has allegedly claimed ownership of the well. Mr. Brown claimed that he is waiting on Mr. King to sign the form 7 for the transfer of ownership.
0076587462 2015-07-21 Production Wells No Violations On the day of my inspection I found that the crude oil/paraffin contamination had not been cleaned up. I called Clayton King. He claimed that he would be out within the next two days to get it cleaned up.

There was a circle of crude oil contamination approximately 8 to 10 in diameter. I called Clayton King to report the leak. He said he would be out to start cleaning it up.
0229583183 2018-04-11 Plug / Plug Back No Violations On the day of my inspection I found that Berry Well Service had rigged down and moved all of their equipment out, except one bulldozer and one track-hoe that were parked at the access road entrance. The well site was rough graded to the approximate contour of the surrounding landscape. The only oilfield related materials left at the site was 2 feet of yellow plastic 1 inch diameter gas line protruding from the ground. The line was used by Clayton King when he owned the well. I called Clayton to ask about removing the line, but he didnt answer. I spoke with Donny Strange of Knox Energy/ Berry Well Service and he said that the cement had fallen a little over night. Donny claimed that it took 13 bags of sack cement to top it off before they cut, capped and buried the casing.
0386042156 2013-05-21 Production Wells Violations Noted I received a call from Robert Kukla (803-528-9233) in regards to an idle well on his property. On the day of my inspection I found that the well head was completely overgrown with multiflora rose. The well head was still equipped with rods, although the pumping unit was disconnected. There was no motor on the pumping unit and it had been moved approximately 15 feet from the well head. There were newer plastic one inch lines hooked up to the well head. I found no production equipment and no identification. ODNR records show that no production has ever been reported for this well. In its current state this well is incapable of production. Make well capable of commercial production within 30 days; or plugging is required within 90 days. Failure to comply may result in forfeiture of bond.

I called Donny Strange with Knox Energy and he said that they are waiting for the ground to dry up a little before they come back in. I also told Donny that I had spoke with the previous well owner, Clayton king, who said that he believed there was a hole in the production casing. Donny said that he would keep an eye on it when they were pulling the tubing.

0740527965 2018-07-20 Final Restoration No Violations On the day of my inspection I found that the drill site and access road were graded, plowed, planted and growing in soy beans. There were no materials or debris left behind and there were no signs that the plugged well was leaking. There were no signs of erosion. The tanks are still in use for permits 2464, 2465 and 3709. This site passes final restoration.

There were belts on the pulley and the gearbox. The bridle was connected. The valve on the casing was closed. The valve on the tubing was open. I saw no pressure on the gauge. The lines at the wellhead were connected. The was also 1 yellow plastic gas line connected. I called Clayton King to come out and start the well. He said he could meet me at 13:00 on 6/24/15 to demonstrate that the well is capable of pumping. There was still no identification at the wellhead or tank battery.

The engine and drive belts were intact. The flow lines were connected at the wellhead. The valves were closed. There was identification written on the pumping unit. I did not find any leaks or spills at the wellhead. There are no existing structures on this property to receive domestic gas. I spoke with the owner Clayton King of Royalty Enterprises and he said that he used to get gas off of the well, but he has not been able to pump it due to legal matters. The well is currently idle.

1268120185 2018-04-10 Plug / Plug Back No Violations On the day of my inspection I found that Berry Well Service has 1.5 inch diameter tubing ran to a depth of 2371 feet for the Big Lime plug. Cemenco hooked up and started pumping to establish circulation. The tubing immediately pressured up to 800 pounds. Cemenco gave it some time to monitor the pressure for the obstruction to be cleared. The pressure decrease was minimal, so Berry pulled the tubing and found that the bottom joint was plugged with downhole formation material. They cleared the obstruction and ran the tubing back in, leaving two joints out for a depth of 2305 feet. Cemenco started pumping and the tubing pressured up again. Berry removed one joint at a time while Cemenco continued to attempt circulation. Berry pulled a total of five more joints for a depth of 2140 feet when the fluid started to move. Berry worked the tubing up and down while Cemenco pumped until they had good circulation. The rig operator, cement engineer and myself suspected that the hole had caved in above the ripped 4.5 inch casing top. I allowed them to set a 250 foot Big Lime isolation plug where they finally achieved good circulation. Berry Well Service and Cemenco also set 200 foot cement plugs through the Berea, at the base of the surface casing and at the surface. Circulation was established and maintained through all of the plugs, with cement being circulated to surface on the final plug. The cement had fallen to approximately 50 feet by the time the rig crew had finished cleaning up. Cemenco topped it off and Berry left the casing open over night.

1398358457 2014-04-24 Production Wells No Violations FOLLOW UP TO CA #386042156: This inspection pertains to complaint #4331. On the day of my inspection I met with Robert Kukla at the well and took updated photographs. I found that the well was still sitting idle equipped with a stuffing box and polished rod. The pumping unit was still disconnected. The valves were closed and the gauge showed 40 lbs. on the tubing. There was new yellow plastic gas line connected to this well that entered the ground in an easterly direction. There were no storage tanks and no identification.

1523025492 2018-04-05 Plug / Plug Back No Violations On the day of my inspection I found that Berry Well Service has the rods out and they were finishing up with pulling the tubing. Donny Strange said that the fluid was up to approximately 200 feet, so he was certain the production casing did have a hole in it. They were going to bring a sting of 1.5 inch diameter tubing back in to use for setting the cement plugs. Donny said that they will be ready to set the bottom plug tomorrow around noon.
1523044343 2018-04-06 Plug / Plug Back No Violations On the day of my inspection I found that Berry Well Service was running the 1.5 inch tubing in the well. They ran the tubing to a depth of 3353 feet. Cemenco established circulation into the steel half-round tank and then pumped a 30 sack cement plug through the Clinton sand.
1523287184 2018-04-09 Plug / Plug Back No Violations On the day of my inspection I found that Berry Well Service had the 1.5 inch diameter tubing out and they were running in the 4.5 inch diameter casing with their ripping tools. The tools kept getting hung up at a depth of 1300 feet. They removed the knife from the tools and were able to get through the tight spot. Donny Strange was operating the service rig. He started looking for a telegraph around 2800 feet and came up the hole in 50 foot increments until they finally got a good telegraph at a depth of 2357 feet. Donny parted the casing there and they started removing it from t he well. They plan to remove all of the 4.5 inch casing and then run back in with the 1.5 inch tubing to set the rest of the cement plugs.

He claimed that H&D Drilling was scheduled to be there on 1/13/15.
