0364801606 2014-07-31 Plug / Plug Back No Violations On the day of my inspection I found that Randy Moran had ripped the 8.625 casing at 390 and pulled the casing from the well. They set a pre-cast cement plug on the casing at 389 and washed 8 tons of clay down the well to fill it to surface. They claimed they would let the clay settle for a couple of days before returning to top it off and close it.
0402764678 2016-10-12 Final Restoration No Violations On the day of my inspection I escorted surveyor Tom Smith to the plugged well site in order to mark the location with a flagged stake at the final recorded GPS coordinates. I found that the vegetation was well established and there were no signs of erosion. There was no evidence that the plugged well was leaking.
0460781665 2015-05-05 Final Restoration No Violations On the day of my inspection I found that the site had been graded out. The storage tank had been removed. The site had minimal grass growth and there was a section of orange gas line above ground. The lease road was not reclaimed. I tried to call Fortune Petroleum. The number was wrong. I called Eric Ashcraft and left a message. He returned my call and I let him know that the site needs to be re-seeded, the gas line removed and a landowner waiver signed to leave the lease road in place.
0681303912 2014-07-24 Plug / Plug Back No Violations On the day of my inspection I found that Randy Moran was rigged up to plug the well. He was pulling rods and tubing.
0710764442 2015-06-23 Final Restoration Violations Noted On the day of my inspection I found sparse vegetation growing at the well site. Approximately 40% of the site was still bare. I found a section of orange plastic 1 line and some 2 flow line left at the site. There was one erosion rut forming from rainwater running off of the site. The lease road was still in place. I contacted Eric Ashcraft on 5/5/2015 to let him know that the site needs re-seeded, the line needs to be removed and they would need a landowner waiver (form 5) to leave the lease road in place. The final restoration for this site was due on 2/11/2015. I called Eric Ashcraft again. He claimed that the weather was preventing them from getting the work done. I gave him 30 days to finish the restoration.

1406731161 2014-07-29 Plug / Plug Back No Violations On the day of my inspection Randy Moran washed 2.5 tons of clay down the 4.5 casing. They ripped the casing at 2780, pulled 10 joints and washed another 2.5 tons of clay down the well.
1437486414 2015-07-20 Final Restoration No Violations On the day of my inspection I found that the site had been re-graded. I spoke with Eric Ashcraft, who claimed that he would be out to seed the site in two days. He also claimed that the landowner wants to keep the lease road. I told him the he will need to submit a landowner waiver (form 5) to leave the road.
1474826547 2013-10-28 Production Wells Violations Noted On the day of my inspection I found that the well head was equipped for a pumping unit that has been removed. The tank battery consisted of two 100 barrel stock tanks and an earthen dike. Proper identification was posted and there were no leaks or spills. In its current state this well is incapable of production. Make well capable of commercial production within 30 days; or plugging is required within 90 days. Failure to comply may result in forfeiture of bond.
1524803144 2015-08-20 Final Restoration No Violations On the day of my inspection I found that the site had been seeded and it was growing well. The segments of flow line had been removed. The road was still in place and there was still a section of exposed gas line from the bottom of the lease road that crossed the creek and went up to the old tank battery site. Eric Ashcraft claimed that the gas line was installed by and belonged to the landowner. I emailed him a copy of the landowner waiver.

1679439004 2014-04-10 Production Wells No Violations FOLLOW UP TO CA # -1474826547: On the day of my inspection I found that nothing has been done to make this well capable of production. The well is still sitting idle with a stuffing box and polished rod. The valves were closed and there was 0 psi on the casing and tubing. The well has been transferred from Ashcraft Petroleum to Fortune Petroleum. This violation is not resolved. I tried to make contact with the company by phone. No one answered so I left a message.
2041981073 2014-07-30 Plug / Plug Back No Violations On the day of my inspection I found that Randy Moran had pulled the remaining 4.5 casing from the well. They set a pre-cast cement plug on the 7 seat at 942 and washed 4 tons of clay down the well to cover the Berea.
