0213415371 | 2015-09-01 | Production Wells | No Violations | On the day of my inspection I found that the wellhead was equipped with a pumping unit. The valves on the 4.5 casing were closed, the valve on the tubing was open. The gauges did not show any gas pressure. The electric motor was intact, but the entire pumping unit was heavily overgrown with multiflora rose and sapling trees. The polished rod was dry and rusted. I found a production chart labeled McGrady that was dated 8/13/15. The chart showed no gas production. This well must be plugged by 9/27/15. There has been no plugging permit issued to date. |
0386070867 | 2017-06-01 | Production Wells | No Violations | On the day of my inspection I found that the access road leading back to the wellhead was no longer visible and there were no signs of recent vehicle traffic back to the well. The flow lines were connected at the wellhead. The tubing valve was open to 2 inch diameter steel flow line that runs underground to a 100 barrel steel tank. I tapped on the side of the tank and it appeared to be empty. The casing valves were open to new 1.5 inch diameter plastic gas line that runs to a small compressor near the tank. The compressor was operating at the time of inspection. I called Dale Smith with M&R Investments to ask about the well. He said that he has notified M&R that the well needs a pump change, but they will not approve a service rig. Dale claimed that the pumper shuts the well in until it pressures up with gas and then opens it to the compressor for sales. The compressor is shared with permit 4604 and the gas production is metered after the compressor as a combination from both wells. Dale did not know the volume of gas off of this well, or the current fluid level. There has only been 10 mcf gas reported for this well since 2011 and no production has been reported for 2016. I suggested that they install a meter to accurately measure the gas production from this individual well. Dale said that he could set a house meter fairly easy and he would contact me when they were installing it. |

1040481634 | 2015-03-26 | Production Wells | Violations Noted | On the day of my inspection I found that the wellhead was equipped with a pumping unit. The valves on the 4.5 casing were closed, the valve on the tubing was open. The gauges did not show any gas pressure. The electric motor was intact, but the entire pumping unit was heavily overgrown with multiflora rose and sapling trees. The polished rod was dry and rusted. There has been no production reported since 2011. The chart at the meter was dated Nov. 2014 and showed no production. This well is idle and must be plugged within 180 days of the issuance of this compliance notice. |
1094623488 | 2017-06-09 | Production Wells | No Violations | On the day of my inspection I found that the house gas meter showed 0017 cubic feet of gas production over the course of three days, which calculates to 2.068 MCF (thousand cubic feet) per year. The casing pressure was down to zero (0) psi and the line pressure at the house gas meter was down to 5 psi. The well is making gas and the compliance notice is resolved. |
1100799451 | 2004-11-18 | Annular Disposal | Violations Noted | AD not hooked up . Idle well , motor removed. Last production reported 1994 . 1/27/05 call from Roy Miller , notified he was going to apply for permit to plug . |

1453575488 | 2015-10-23 | Production Wells | No Violations | On the day of my inspection I found that the pumping unit was still heavily overgrown with vegetation. The valves were closed and there was no pressure on the gauges. The polished rod was still dry and rusted. There was a new chart dated 10/8/15 which showed zero (0) mcf gas production. I called M&R Investments and spoke with the secretary. She took my information and said that someone would be getting in touch with me. I was contacted by Gene Varner, who claimed that the well was capable of production and it was making house gas. I told him that house gas does not qualify as commercial production, and they either need to get in there and service the well to make it capable or plug it. |
1470233417 | 2016-07-28 | Administrative Inspection | No Violations | I received a phone call from pumper Gene Varner to notify me that the gas line had been run to the well and connected. |
1470925093 | 2016-08-11 | Production Wells | No Violations | On the day of my inspection I found that the new plastic gas line had been connected at the wellhead. The flow lines were connected to the tubing and the casing. The tubing valve was open. Both valves on the flow line from the casing were open. There was a gauge on the casing flow line that showed 300 psi. but the gauge was missing the glass face cover. The electric was connected at the pole and to the motor. All of the vegetation had been cleared from around the pumping unit. There was chart in the meter box that showed some production, but the chart was unlabeled with no date. |
1480444547 | 2017-06-06 | Production Wells | No Violations | On the day of my inspection I found that there was a new house gas meter installed at the wellhead, between the steel 2 inch diameter casing flow line and the 1.5 inch plastic gas line. The casing gauge showed 30 psi and the gauge between the regulator and the meter showed 13 psi. The valves were open and I could hear gas coursing through the line. The 1/2 foot and 2 foot dials were turning on the meter, and the main reading was at 0004 cubic feet. |
1481126141 | 2016-12-06 | Production Wells | No Violations | On the day of my inspection I found that there were no changes at the well. The tubing valve was still open, but the appearance of the polished rod gave no indication that the well had been pumping. The chart near the tank and compressor was not labeled or dated, and it did not show any gas production. I called Dale Smith with M&R who said that the fresh water treatments did not work and he could still not get the well to pump. He claimed that they were selling gas off of the well, but the well needs to be serviced. He said that he would be attending a meeting with Tim Altiers tomorrow and he would discuss the need to get a service rig on the well. He said that he would get back to me. |
1850233763 | 2016-07-25 | Production Wells | No Violations | On the day of my inspection I met with pumper Gene Varner at the well site. The pumping unit was still overgrown with small trees and weeds. The valves were closed and the polished rod was dry. Gene claimed that the well was capable of selling gas and he cracked a valve on the casing to demonstrate that there was pressure. The gas line had allegedly been damaged from farming activity. Gene said that he and Dale Smith had been out to lay new line. I did observe a path of freshly disturbed soil from the tank battery to the edge of the tree line, and a new plastic line that was capped at the tree line. Gene claimed that they still needed to run the line down to the well. He said that they would have the line connected within the next week, and they planned to have a service rig in as soon as possible. Gene claimed that the rods were parted down hole and would need to be serviced to make the well capable of pumping. |
2079472769 | 2016-10-14 | Production Wells | No Violations | On the day of my inspection I found that the valve was open on the casing flow line that connected to the new gas line. The valve was also open on the tubing flow line. The polished rod and stuffing box were dry, and the walking beam on the pumping unit was still in the same position as it was on prior inspections. The electric motor was mounted, the drive belts were in place and the electric was connected. There was approximately 3 feet of fluid in the 100 barrel tank. The new gas line was connected to a drip bottle which then ran to a compressor. The valves were open on the drip line, but the gauge showed (0) zero psi. There was an unlabeled chart in the box which showed no production. There was 10 mcf gas production reported for 2015. I called Dale Smith, field superintendent for M&R Investments. He said that he believes the well has a salt ring, and he has just got clearance to contract a pump truck to start addressing these types of issues. Dale said that he would make this well a priority job, and he would be out within the next two weeks to treat the well and try to get it pumping. I told Dale that if the water/solvent treatment does not work they will have to bring in a service rig. |
