0034331831 2016-09-13 Production Wells No Violations On the day of my inspection I found that the area outside of the dike was dry, clean and covered in a layer of fresh soil. The dike walls had been built up and compacted.
0181698379 2012-08-08 Production Wells Violations Noted On the day of my inspection I found that the well was equipped with a pumping unit and connected to a tank battery that consisted of one 100 barrel stock tank, one 30 barrel brine storage tank and an earthen dike. There was a 2 wide streak of oil down the side of the stock tank, approximately 60% of the ground inside the dike was covered with standing crude oil, which was covered with straw, and there was an area outside of the dike approximately 18 x 20 contaminated with standing crude oil, which was also covered with straw. I was unsuccessful at reaching the well owner, as the emergency contact number in the ODNR database is no longer in service. All contamination inside and outside of the dike must be removed immediately and a current emergency contact number must be posted at the tank battery.

1033129366 2016-06-09 Administrative Inspection No Violations I called well owner Jim Vieira to discuss the issues at the tank battery. I did not get an answer so I left a voice message. Jim called me back and claimed that he had no knowledge of fluid being drained into the dike. I explained what the issues were and what actions are needed to bring the well into compliance. Mr. Vieira said that he could have someone out to drain the fluid and remove the soil by the first of next week. He also agreed to install locks on the drain valves at the tank. He requested permission to bio-remediate the impacted soil on site with lime and fertilizer. I discussed the proposal with Emergency Operations Coordinator Josh Schiering. We agreed to allow Mr. Vieira to bio-remediate on site, under the condition that I be present to witness the lime and fertilizer being mixed with the soil.

1465913782 2016-06-14 Production Wells No Violations On the day of my inspection I found that the clean up of the oil in the dike had not been started. I called Mr. Vieira and he said that he would be out this afternoon with an excavator to start the clean-up. I met Mr. Vieira at the well site later in the day. He had an excavator on site. The excavator scraped up the bare patches of soil outside of the dike and Mr. Vieira mixed in one bag of granulated lime and one bag of fertilizer. When I left the location the oil impacted gravel and soil was being removed from the dike. Mr. Vieira was treating the soil with lime and fertilizer. The treated soil and gravel was to be mixed with fresh soil and spread at the base of an existing stockpile of soil that Mr. Vieira had near the well site. I also discussed modifying the dike with Mr. Vieira. Much of the low, wet area could be eliminated.
1465998594 2016-06-15 Production Wells No Violations On the day of my inspection I found that the oil impacted soil and gravel had been removed from inside the dike. There were still small spots of oil floating on the water that was left behind in the low wet area. Lime and fertilizer had been spread across the inside of the dike. The impacted soil and gravel was mixed in with fresh soil and spread at the base of an existing soil stock pile. The bare areas outside the dike had been churned up and mixed with lime and fertilizer. I called Mr. Vieira to ask what his plans were for dike. He claimed that he was bringing his excavator back in to rework the dike and bring in new gravel. He said that he would have it done before 7/8/16.
1468330189 2016-07-12 Production Wells No Violations On the day of my inspection I found that the dike had been rebuilt to about half the original size. A PVC drain had been installed with a plug. There was no standing fluid or contamination inside of the dike. The three small areas outside of the dike had been treated with lime and fertilizer, and mixed in place. The 2 plastic line from the 100 barrel tank was running to the hatch on the 50 barrel tank. The dike still needs work to prevent fluid loss, but the contamination has been cleaned up.
1470837063 2016-08-09 Production Wells No Violations I was contacted by Emergency Operations Regional Manager Trisha Cartmell to request that I meet her at the well site to inspect the clean-up of the tank battery. We did find some minor staining in the soil where the dike had been re-worked. There was no standing fluid in the dike and the drain was equipped with a plug. Trisha said that she was closing the case with regards to pollution and contamination; however, she requested that I contact Mr. Vieira to ask that he improve the dike berms.
1471614087 2016-08-12 Administrative Inspection No Violations I called Jim Vieira to request that he treat the remaining oil stained soil outside of the dike with lime and fertilizer. I also asked Jim to come back in to reinforce the dike walls by shaping and compacting them to better hold fluid. Jim said that he would be out this weekend to treat the stained soil, and he would bring the contractor back out to work on the dike walls.

1558882794 2016-06-08 Production Wells Violations Noted On the day of my inspection I noticed what appeared to be oil in the dike while driving by. I stopped to investigate and found that approximately half of the dike contained fluid. There was a 4 to 5 foot diameter pool of crude oil which spread into water that was covered in an oily sheen. There were no apparent leaks from either the 100 barrel tank or the 50 barrel tank. There was an orange plastic line connected to the drain valve on the bottom of the 100 barrel tank that extended in the direction of the oil. The line was open ended and there was crude oil residue in the opening. I conducted a field test on the water with Quantab test strip and found the chloride level to be around 900 parts per million. There were also three bare areas outside of the dike where the soil exhibited heavy cracking, oil stains and deer tracks. I tried to mix a sample of the soil with some water to get a Quantab reading. The soil had a strong odor that was very similar to firearm lubricant. I could not get the Quantab to titrate, but the portion that did absorb the soil mixed water showed the presence of chlorides. All crude oil and brine water must be removed from inside of the dike. All crude oil and brine impacted soil must be removed from inside the dike and from the field next to the dike. The soil must be hauled to a landfill and a copy of the manifest must be provided.

There was one 100 barrel steel tank and one 50 barrel steel tank in use for fluid storage. There was an earthen dike in place around the storage tanks. There were no leaks or spills around the storage tanks.
1624343108 2016-08-19 Production Wells No Violations On the day of my inspection I found that the recently excavated area outside the dike was under standing water. The area has been subjected to heavy rains over the past week. The far end of the dike has historically been prone to holding rain water year round, which was a contributing factor in suggesting that Mr. Vieira reduce the perimeter area of the dike in order to avoid the standing water. Now that the dike walls have been reduced in diameter the area is still saturated with standing water, although it is all outside of the dike walls. The inside of the dike was free of any standing water. The issue now is that the standing water outside of the dike is discolored and there are some small pockets of floating sheen. There have been documented releases inside of the dike in the past. The soil has likely been subjected to oil and brine over the years that it was part of the containment dike, and now the rain water is picking up low levels of contamination. I conducted a field test on the water with a Quantab test strip and found the chloride level to be at 447 parts per million. There were no current leaks or spills in the dike. I called and left a message with Mr. Vieira to set up a time when he could meet me at the well site. Jim Vieira returned my call. He claimed that he would have the excavator come back out to remove more of the soil outside of the dike. Then he would treat the area with lime and fertilizer and spread fresh soil over the area. He also claimed that he would he would reinforce the dike walls.

1835225141 2016-07-13 Production Wells No Violations On the day of my inspection I met with Trisha Cartmell and Josh Schiering from the Emergency Response section. The contamination had been cleaned up, but the dike construction still needed work. The dike walls were very loosely laid with multiple cracks and crevices. The walls need to be compacted in order to prevent the escape of fluids.
1903209290 2012-09-14 Production Wells No Violations ON the day of my inspection I found that the contaminated soil had been removed from inside the dike and peat had been spread. There was still some residual oil in the soil outside the dike. I called Jim Vieira and instructed him to remove the peat and any more contaminated soil from inside the dike, and I recommended spreading gravel in the dike. Mr. Vieira was also instructed to remove the contaminated soil outside of the dike and to post proper identification at the tank battery.
1989130263 2012-08-27 Production Wells No Violations ON the day of my inspection I found that most of the contaminated soil had been removed from inside the dike. The contaminated soil outside of the dike was also removed. I received a call from Jim Vieira (614-989-6477) on 8/25/12 to inform me that he was removing the soil to be mixed with lime and fertilizer and spread on the property, and to request an extension on the clean up. The site still needs some work. I granted a two week extension. This violation continues.
2017116148 2012-09-19 Production Wells No Violations ON the day of my inspection I found that all of the oil soaked peat and soil had been removed inside and outside of the dike. A layer of gravel was spread inside the dike and identification was posted. No violations at this time.
