2018-06-26 | Plug / Plug Back | No Violations | On the day of my inspection I found that the clay pile was gone and the well was covered with soil. I called Phil Moran to ask for an update. Phil said that he was at the site yesterday and he found that the clay had settled downhole over the weekend. He said that they had to put another 4 tons in to fill the well to surface. Phil said that once the well was filled to surface he cut, capped and buried it.
2011-04-06 | Production Wells | No Violations | Gas still venting thru 2 valve at well head. No change from last inspection. Will call company back.

2019-05-17 | Final Restoration | No Violations | On the day of my inspection I found that the site and access road still needed a final grade and they needed to be seeded for vegetative growth. I called Dwayne Dugan to remind him that this site still does not meet the criteria for final restoration. He said that he would have his crew out to get started.

2011-03-07 | Production Wells | No Violations | Complaint of gas venting at well head, Inspection of well showed 2 valve open on casing at well head. Gas is being vented without a pressure relief valve. A flare 100 from well head is disconnected at well head. Well must be equipped to prevent continues venting of gas.
2018-06-13 | Plug / Plug Back | No Violations | On the day of my inspection I found that Phil Moran was rigged up on the well, and he had the rods and tubing pulled. There was a load of clay from Cedar Heights that had been dumped on the side of the road and covered with a tarp. Phil was not working at the site. When I spoke with him he was unsure if he would be out today.
2011-05-27 | Production Wells | No Violations | Line hooked up at well head then runs approxiamtly 100 and is hooked up to a flare. Compliance.
2011-03-07 | Production Wells | Violations Noted | Construct well head to prevent continues venting of gas. ( Flare, shut in or install a properly operating relief valve ).
2018-06-14 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran washed 2 tons of clay down the 4.5 inch diameter casing. Then he went in with ripping tools to part the casing. Phil found a good telegraph at a depth of 3145 feet and parted the casing there. He pulled the string up 10 feet and shut down for the day.
2018-06-19 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran ran a pre-cast cement plug in to the 7 inch casing seat at a depth of 1078 feet. The tools were covered with oil when Phil came back out of the hole, so we both agreed that he would need to bail the hole before he attempted to pull the surface casing. Phil got his 8 inch bailer, dug a small pit and lined it. It took three runs with the bailer before he had all of the oil out, and started to bring water to the surface. Phil equipped his 8 inch spear and got ahold of the surface casing. He attempted to pull but there was no stretch or movement in the pipe. Phil locked the rig in with tension on the casing and shut down to let it sit over night.
2018-06-20 | Plug / Plug Back | No Violations | On the day of my inspection I found that the surface casing had not moved after sitting overnight under 25000 pounds of tension. Phil Moran started searching for a telegraph at 500 feet and found one at 154 feet. Phil parted the 8.625 inch diameter casing at 154 feet and pulled it from the well.

2018-06-15 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran pulled 20 joints of the 4.5 inch diameter casing out of the well and then he washed another 6 tons of clay in, for a total of 8 tons on the bottom plug. Phil started pulling the remaining 4.5 inch casing from the well. He planned to run the pre-cast cement plug on Monday and start working on pulling the surface casing.
2018-06-21 | Plug / Plug Back | No Violations | On the day of my inspection Phil Moran rigged down and moved most of his equipment out before running any clay. Phil washed clay in the well until it was filled to the surface. He got approximately 8 tons on the pre-cast cement plug at 1078 feet before the hole filled up. Phil said that he was going to leave the well open over the weekend to allow adequate time for the clay to settle. We will check back at the first of the week.
