0365761632 2016-04-06 Production Wells No Violations On the day of my inspection I found that the oil soaked peat and soil had been dug up and removed from inside the dike. There was a shallow hole dug that had collected rainwater and a skim of residual oil. I knocked on the outside of the tank in order to get a rough estimate of the fluid level. The tank appeared to be empty.

0462778621 2016-04-22 Production Wells No Violations On the day of my inspection I found that the clean up of the crude oil in the dike had not been completed. I called David Bonar and his pumper Jerry Addy and told them both that the remainder of the oil must be removed and the impacted soil must be neutralized with granulated lime and fertilizer. I also told them that the Jones 1& 2 wells must be shut in until they assume legal ownership.

0790436856 2016-03-22 Production Wells Violations Noted On the day of my inspection I found that the well was being produced with a plunger lift system. The tubing valve was open. One of the casing valves was open and the other was closed. There was pressure on the gauges. There was a vertical separator back at the well site. There was pressure on the gauge and the valve was open. The tank battery consisted of one 100 barrel steel tank and one 20 barrel poly tank. The poly tank was cracked open on the top and didnt appear to be in use. An earthen dike was in place. The elbow on the 2 riser for the 100 barrel tank was leaking. There was oil splattered up the side of the tank and there was a 5 foot semi-circle of oil of crude oil on the ground around the tank. No oil had escaped the dike. There was identification posted on the tank battery for Poorboy Exploration, and there was a chart that was dated 3/17/16 for the Jones 1 & 2 which showed production. No production has ever been reported for this well. I called David Bonar and met with his pumper Jerry Addy, who both openly admitted to producing this well. I told Jerry that the well had to be shut in until the ownership was straightened out. I also told him that the leak must be addressed and the oil in the dike must be cleaned up.
0860824168 2016-03-30 Production Wells No Violations On the day of my inspection I found that the oil had been cleaned off of the tank and the 2 elbow had been replaced. The oil on the ground inside the dike had been covered with peat. There was a wet spot on the bottom of the tank and the peat under the spot was saturated with oil. I called David Bonar and I told him that the tank is still leaking and must be addressed. I told him the oil and peat inside the dike still needs to be removed. I also reminded him that both Jones 1 and 2 wells must be shut in until the he has assumed rightful ownership.

1362753411 2013-03-08 Production Wells No Violations FOLLOW UP TO NOV # 913224600: On the day of my inspection I found that the well head was equipped with a plunger lift system and there was a separator in place near the well head. There were no leaks or spills at the well head or separator. The tank battery consisted of one 100 barrel stock tank, one 20 barrel poly brine tank and an earthen dike. The stock tank was leaking oil from the 2 draw valve. The brine tank was cracked open at the top. There was no identification posted, and no statement of annual production. Nothing has been done to correct the violations issued in 2008.

1462984892 2016-05-11 Production Wells No Violations On the day of my inspection I found that the oil impacted soil inside of the dike had not been cleaned up. The hole that was collecting water and oil was still in place. There was a new pool of oil forming under the heater tube. The 20 barrel poly tank was cracked in the rear and there was a small amount of staining on the ground under the crack. I called David Bonar of Poorboy Exploration. He said that someone would be out to look at it today and then he hung up on me.
1463152777 2016-05-12 Production Wells No Violations On the day of my inspection I was accompanied by Southeast Region District Supervisor Mike Moreland. We found that a five gallon bucket had been placed under the heater tube opening to catch the oil that was leaking. The oil impacted soil and gravel around the tank was still in place and there was still a 3 foot diameter hole that had been dug in the dike that was full of water and crude oil. The tank appeared to be leaking from the bottom seam as well as the heater tube. There was also a cracked 20 barrel poly tank in the dike that still needs to be removed. We walked back to the wellhead and found that the valves were open on the tubing and casing. The valve leading to the separator was closed and the valve leading from the separator to the flow line was closed. There were no leaks at the wellhead or separator. I called Emergency Operations Coordinator Josh Schiering to report the contamination. He met us at the site to collect photos and begin his investigation. David Bonar of Poorboy Exploration was issued a compliance notice on 3/22/2016 via phone call. I also met his pumper Jerry Addy in person regarding the compliance issues. The 100 barrel steel tank and the 20 barrel poly tank must be removed from the dike. All contaminated soil and gravel must be excavated from inside the dike and hauled to an approved landfill, and a copy of the manifest must be provided.
1491558499 2016-05-23 Production Wells No Violations On the day of my inspection I found that the five gallon bucket had been removed and the heater tube had been plugged with an expanding plastic plug. The tank was still leaking from the bottom seam and there was still standing crude oil in the dike around the tank. The hole full of water and oil was still in place and the 20 barrel poly tank was still in place.
1491924475 2017-04-11 Production Wells No Violations On the day of my inspection I found that the remediated tank battery site was 90% covered in grass. I found no signs of residual oil contamination from the tank leak last year. The only oilfield related materials left behind at the site was a steel tank ladder. The access road entrance had been altered into a gated driveway for a cabin and camper. I called David Bonar with Poorboy to ask about his progress with assuming ownership of the Jones wells. He did not answer so I left a message.

1688699334 2016-06-06 Production Wells No Violations On the day of my inspection I found that David Bonar of Poorboy Exploration was at the tank battery with an excavator and a water truck. The 20 barrel poly tank had been removed. He was draining the 100 barrel steel tank in preparation to be removed as well. Mr. Bonar claimed that he was removing the production equipment and he would not replace it until the wells were under his ownership. He said that he would be removing the contaminated soil once the equipment was out. He also claimed that he was working with Jon Austin to assume ownership and he was posting an affidavit of forfeiture in the local paper concerning the Jones 1 and 2 wells.

2094260703 2016-06-09 Production Wells No Violations On the day of my inspection I met Emergency Operations Coordinator Josh Schiering at the well site. The 100 barrel tank and the 20 barrel poly tank had been removed. The dike had been removed and the area was also graded out, seeded and covered in straw. The well was shut in at the separator back at the wellhead. I called David Bonar about the contaminated soil and he claimed that he took 27 or 28 tons of soil to the Athens-Hocking Reclamation Center landfill. This violation is resolved.
