0179685770 2012-10-25 Production Wells No Violations On the day of my inspection I found that the well head was equipped with a plunger lift system and connected to a tank battery that consisted of one 210 barrel stock tank, one separator and an earthen dike. Proper identification was posted and there were no leaks or spills at the well head or tank battery. No violations at this time.
0326860216 2017-09-08 Final Restoration No Violations I visited the site with Tom Smith to survey the plugged well location. I found that the tank battery was still in place and the well casing was still visible above ground. The casing was still filled with cement to the surface. All other equipment and debris had been removed from the site. I later spoke with Dave Gross, who said they would be out this week to cut the casing off and cap it, and remove the production equipment.
0913260055 2018-06-07 Final Restoration No Violations On the day of my inspection I found that the gravel access road was still in place and in good, passable condition. The well site was 80 percent covered with grasses and broadleaf vegetation, and there were no signs of erosion. I found no oilfield related materials or debris left at the site, and I found no signs that the plugged well was leaking. I called Red Hill to ask about the access road. I recommended that they get a landowner waiver (form 5) signed if the landowner wants to leave the road in place.

1249044456 2018-09-19 Final Restoration No Violations On the day of my inspection I found that the access road was still in place and clear of vegetation. There were no erosion issues with the road. I have not received a landowner waiver and I have not received any communication from Red Hill since my last inspection, when I called Chris Kimble on the access road. I called him again and he said that they have been in touch with the landowner. Chris said that he expects to get the signed waiver back within the next week. I asked that he get me a copy once the have the form back.

1336045411 2019-02-07 Final Restoration No Violations On the day of my inspection I found that the former well site was still in good condition with established vegetative cover , and no erosion issues. The access road was also in good condition. There were no ruts or signs of erosion. I received a signed landowner waiver (form 5) from Red Hill to leave the access road in place. This site passes final restoration.

1502806478 2017-08-15 Plug / Plug Back No Violations On the day of my inspection I found that Bakerwell was rigged up to plug the well. There was one open top frac tank on site to circulate into and one 500 barrel frac tank for water. The well was shut in and no one from Bakerwell or Redhill was present.
1502887687 2017-08-16 Plug / Plug Back No Violations On the day of my inspection Petroset hooked up to the 4.5 inch diameter casing to bullhead the bottom plug. They pumped 30 sacks of class A cement with 2% gel and cotton seed hulls. The well went on vacuum during displacement, so Petroset shut the pump truck down 10 barrels short of displacement to let the well draw the cement into the perforations. Once the full 63 barrels of displacement was reached they shut everything in for one hour to allow the cement to begin to set. Mark with Petroset opened the valves after an hour and the vacuum had stopped. Bakerwell ripped the 4.5 inch diameter casing at a depth of 3300 feet and pulled up to 3057 feet to set the Big Lime plug. Petroset established circulation and pumped a 200 foot cement plug in the top of the Lime. Bakerwell pulled up to 1186 feet for the plug at the base of the surface casing. Petroset established circulation and pumped a 200 foot cement plug at the base of the surface casing. Bakerwell then pulled the remaining 4.5 inch casing from the well. Appalachian Well Survey set an electric bridge plug at a depth of 950 feet. Bakerwell ran five joints of 4.5 inch casing back in the well to set a surface plug. Petroset pumped until cement was circulated to surface.
1505400764 2017-09-14 Final Restoration No Violations On the day of my inspection I found that the well casing had been cut off and buried. I did see the steel plate during my previous inspection. The production equipment had also been removed. The site was graded out to match the surrounding contour.
