0455974466 2014-06-06 Final Restoration No Violations On the day of my inspection I found that the pit was closed, the location was graded out, seeded and covered in straw. The production equipment had been removed. I found no oil and gas related materials or debris. The lease road is still in use to access agricultural pasture.

They pulled the tubing up off the plug to set a new one. Petroset broke circulation and pumped a 100 plug above the existing Big Lime plug. Bakerwell pulled up to 685 and Petroset pumped a 200 Berea/base of surface casing plug. Bakerwell pulled up to 200 and Petroset pumped till cement was circulated to the surface.

1366385647 2013-04-19 Plug / Plug Back No Violations On the day of my inspection Bakerwell ran in with 4.5 casing, equipped with and overshot. They made a connection with the lower half of the parted 4.5 casing down hole and pulled 10,000 pounds. Appalachian Well Surveys shot the 4.5 casing off at 3451. Bakerwell pulled the 4.5 casing and ran back in the hole with 1.5 tubing to 3434.
1366807210 2013-04-24 Plug / Plug Back No Violations On the day of my inspection I found that the cellar was full of rain water and the top of the well casing was not visible. I did observe a small steady stream of bubbles emanating from the center of the pooled water. I used a hand-held gas detector to sample the air just above the water at the bubble stream. I got low readings of CH4 and CO, but nothing significant enough to trip the detectors alarm. I contacted Bakerwell to request that they have the cellar dry and the casing exposed for further inspection on 4/29/13.
1368107174 2013-05-09 Plug / Plug Back No Violations I spoke with Sonny of Bakerwell on 5/8/13, who claimed the well had been topped off with sack cement and the leak was stopped. On the day of my inspection I found that the well was still emitting gases. I sampled the air inside of the casing with a PHD6 hand-held gas detector. The warning alarm was activated and the detector indicated CO - 36, CH4 - 0.45, LEL - 8%.
1377098248 2013-08-21 Plug / Plug Back No Violations I received a phone call from Bakerwell claiming that there were no more signs of gas bubbling from the well. I calibrated my PHD6 hand-held gas detector and equipped it with new filters. When I removed the steel plate from the top of the well I did not observe any bubbling from the fluid in the 8.625 casing. I sampled the air inside of the casing with my gas detector, just above the fluid line. There were no significant readings from inside of the casing. The detectors alarms were not triggered.

I instructed them to cut the casing off below the sack concrete to expose the original class A cement top in order to sample the well with a gas detector. The crew claimed that their excavator was down, and they would contact me as soon as they get it running.

1996098375 2013-04-30 Plug / Plug Back No Violations On the day of my inspection I found that there were intermittent releases of bubbles from the fluid inside the open casing. I sampled the air down in the casing just above the fluid level with a PHD6 hand-held gas detector, equipped with probe. The detectors alarms were set off within 30 seconds of starting the test. The following levels were observed before stopping the test and moving away from the well: 1.36-CH4; 256-CO; 2-H2S; 20%-LEL. I contacted Bakerwell to report my findings. They are aware of the situation and will contact me with a plan to remediate the plug job.
2132814916 2014-05-07 Plug / Plug Back No Violations On the day of my inspection I found that Bakerwell was drilling out the surface plug with a 7.875 tri-cone bit. They are also required to drill out the plug at the base of the surface casing. Once the two plugs are drilled out they are required to run tubing to the top of the existing Big Lime plug. They will set another plug above the Big Lime and new plugs at the surface casing and the surface.
