0092549033 | 2016-07-12 | Administrative Inspection | No Violations | I contacted Bruce Carpenter with eclipse to ask about their plans for plugging the well. Bruce claimed that the plugging contractor, Barry Bowers, was scheduled to move in and rig up on Wed. July 13. |
0339749510 | 2015-12-10 | Production Wells | Violations Noted | On the day of my inspection I found that the wellhead was equipped with tubing and rods. The pumping unit was removed. The valves on the tubing and casing were both closed. This well is idle and not capable of production in its current state. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

0696456522 | 2016-08-10 | Final Restoration | No Violations | On the day of my inspection I found that the flow line was still in place and the site had not been seeded. There were no signs that the plugged well was leaking, and there were no erosion issues. |
0704502804 | 2015-09-16 | Production Wells | No Violations | On the day of my inspection I found that the wellhead was equipped with tubing and rods. The pumping unit had been removed. The valves were closed. The former access road was overgrown with thick vegetation. This well is idle and not capable of production in its current state. I called Dale Smith of Eclipse to ask about their plans for plugging the well. There was a re-issue plugging permit issued on 7/15/2014. He acknowledged that the well was idle and said he would get back to me on its status in the plugging program. |

0980816604 | 2016-07-21 | Plug / Plug Back | No Violations | On the day of my inspection I found that Huffman Bowers Well Service was pulling 2452 of 4.5 casing from the well, and running 1.5 tubing back down to the top of the Big Lime. They projected to be back on Monday, July 25 to set the cement plugs and finish the job. |
1124806559 | 2016-07-14 | Plug / Plug Back | No Violations | On the day of my inspection I found that the rods had been pulled and staged on a trailer. The tubing was still in the well. I met with Eddie Moss of Diversified Resources when he was out to haul the rods away. He claimed that the crew would not be working today. They would be back on Friday to pull tubing and then he would decide if they would steam the tubing on site to be used in the plug job, or bring in a new string. |
1133795933 | 2016-06-01 | Administrative Inspection | No Violations | I was contacted by Bruce Carpenter of Eclipse Resources to request a 30 day extension on the compliance notice to plug the well. Bruce alleged that the well had been sold to Diversified Resources and the purpose of the extension request was to allow adequate time for the transfer to be processed. I granted the extension. |

1450465581 | 2015-12-16 | Administrative Inspection | No Violations | I received email correspondence from Rocky King, EHSR field tech for Eclipse Resources, in response to the compliance notice. The email states that the action plan is to plug and abandon the well before the 6/10/16 due date. |
1468434226 | 2016-07-13 | Plug / Plug Back | No Violations | On the day of my inspection Barry Bowers was moving in and rigging up to plug the well. Rocky King, consultant for Eclipse Resources and Eddie Moss of Diversified Resources were on site. Bowers crew cleared the access road to the well and cleared the well site. They brought the rig in and rigged up. Then they dug the pit and lined it. They started pulling rods and laying them on a trailer. The area around the wellhead and the ditch to the pit were also lined. The paraffin from the rods was contained on the liner. |
1468591756 | 2016-07-15 | Plug / Plug Back | No Violations | On the day of my inspection I found that Barry Bowers crew was on location pulling the tubing form the well. The tubing was packed with paraffin and pulling wet. The water from the tubing was splashing on the ground around the wellhead and rig. I spoke to the rig operator and requested that he use the dozer to push a berm around the wellhead to contain the fluid being brought up with the tubing. Although the pit and the ditch to the pit were lined, the excess fluid was running in the opposite direction. The earthen berm was successful at temporarily containing the water. I wrote a plugging plan for the well and gave a copy to Eddie Moss and the service rig operator. |
1469041461 | 2016-07-20 | Plug / Plug Back | No Violations | On the day of my inspection I found that Huffman Bowers Well Services had 1.5 tubing set at 3755. Barry Bowers pumped a 25 sack cement bottom plug. The rig crew pulled the tubing from the well and went in to rip casing. They informed me that the original 2.375 tubing was badly corroded and deteriorated from 2500 to 2800. They ran in with ripping tools to 3200. They had brief telegraphs, but could not get the tools to set down. They pulled up and made multiple attempts with no success. They crew finally got a solid telegraph at 2452 and got the casing to part at that depth. They pulled one joint and shut down for the night. They packed the wellhead off and capped the 4.5 casing with a swage and valve before leaving. |
1469453132 | 2016-07-25 | Plug / Plug Back | No Violations | On the day of my inspection I found that Huffman Bowers Inc. had 1.5 tubing set at 2425. Barry Bowers pumped a 250 cement isolation plug just above the top of the Big Lime. The service rig crew pulled the tubing up to 1118 and Barry Bowers pumped a 178 cement plug through the Berea sandstone. Then they pulled up to 707 and Barry Bowers pumped a 200 cement plug at the base of the surface casing. Then they pulled up to 200 for the surface plug. Barry Bowers pumped until cement was circulated to the surface. The cement fell back 3 feet down the hole when Barry finished pumping and stayed up. The well had been plugged and abandoned. This violation is resolved. |
1578852493 | 2016-12-05 | Final Restoration | No Violations | On the day of my inspection I found that the well site and access road were covered in short grasses. There were no erosion issues. There were no materials or debris left behind. The tank battery is still in use for permits 6649 and 6650. This site passes final restoration. |
1834126907 | 2016-10-14 | Final Restoration | No Violations | On the day of my inspection I found that the flow line had been removed. I found no other oilfield related materials or debris at the plugged well site. The soil had been final graded and seeded. There was moderate grass growth, with only 50 to 60 percent coverage. There were no issues with erosion. |
1908944857 | 2016-05-24 | Production Wells | No Violations | On the day of my inspection I found that the well was still equipped with tubing and rods, and the valves were closed. I contacted Bruce Carpenter with Eclipse to ask what the plan was for the well. The outstanding compliance notice requires that the well be plugged by 6/10/16. Bruce took the information and said that he would get back to me tomorrow to let me know if the well was included in the transfer to Diversified Resources or retained by Eclipse. |
2006210207 | 2016-08-02 | Plug / Plug Back | No Violations | On the day of my inspection I found that the pit had been closed and the site had been final graded. There were no plugging materials or debris left behind. There was one section of 2 flow line that was still in place along the tree line toward the access road. There was also a dozer left at the site. Eddie Moss of Diversified Resources arrived to pick up the dozer while I was on the site. He claimed that Dale Smith had the casing cut off and capped the well with a steel plate prior to grading the site. I pointed out the flow line to Eddie. He said that they would get it out of there when they were out to seed the site. The tank battery is still in use for permits 6649 and 6650. |
