0525845190 2016-03-07 Production Wells Violations Noted On the day of my inspection I found that the well was being produced with a plunger lift system. The surface casing head was buried and not visible. The production casing valves were closed. The tubing valve was open. The tank battery consisted of one 100 barrel steel tank and one vertical separator. The load valves on the tank were equipped with plugs. The identification sign posted on the tank was correctly filled out and legible. An earthen dike was in place around the production equipment. There was a lot of standing water inside of the dike and there were streaks of iron coloration extending from the tank into the water. I conducted a field test for chlorides with a low range Quantab test strip placed in the water next to the tank. The bar scale showed white to the titration line, indicating a chloride level greater than 664 parts per million (ppm). I tested the water in the same location with a high range Quantab test strip. The bar scale showed white to the titration line, indicating a chloride level greater than 6627 ppm. I also tested the water at the rear of the dike, approximately 10 feet from the tank. The bar scale showed white to 6.8, indicating a chloride level of 2154 ppm. I found no evidence that the fluid had leached through the dike walls or migrated off site. I called Jeff Gress to report the issue. He claimed that he had replaced the tank a year ago because the old tank had given out. I told him that the new tank is leaking or the soil is so saturated with chlorides from a previous leak that the rain water has become contaminated. He claimed that he would call a water truck to empty the tank and the dike, and he would be out to shut the wells in and inspect the tank for leaks. I also told Jeff that tank needs to be moved and all of the contaminated soil inside of the dike must be removed and properly disposed of. He asked if he could spread it on the lease road. I agreed to let him spread it on the lease road under the condition that it must be

1151284080 2016-03-09 Production Wells No Violations On the day of my inspection I found that Jeff Gress was on location with a front-end loader removing the contaminated soil and gravel from inside of the dike. He pulled the 100 barrel steel tank out and had a newer 50 barrel steel tank on site as a replacement. He claimed that the bottom of the tank looked good, but he just wanted to be sure. He had a stack of bagged lime and fertilizer on location to be mixed in with the excavated soil. Jeff told me that he had decided against a dike drain because of the high risk of vandalism, as the wells are located on public recreation area. He claimed that he regularly experiences cases of vandalism and theft at the well sites. We discussed setting the new tank at a more elevated position in the upper corner of the dike in order to avoid water exposure when the dike fails to drain in a timely manner.
1475057453 2016-03-08 Production Wells No Violations Jeff Gress called me on the night of 3/7/16 to report that he had been out with a water truck to empty the tank and inspect it for leaks. On the day of my inspection I found that the dike had been 75% drained and the tank had been emptied. The remaining water in the dike tested positive for chlorides. I also found that the soil around the tank had been dug away to expose the bottom shoulder. There was some rust, but no apparent leaks. I called Jeff Gress and he claimed that he strongly believed that the tank did not have a leak. He admitted again that he had a tank leak there a year ago and estimated that 10 barrels of fluid were released into the dike. I explained to him that I was issuing a compliance notice outlining the issues and I made sure he was aware of the 30 day time frame to correct the issues. He said that he would send water truck back out to suck out the remaining water, and he would be out on 3/9/16 to start the excavation. We agreed that he is to move the tank first, then excavate the contaminated soil and gravel. The soil then must be treated with lime and fertilizer before spreading it on the lease road. Once the dike has been cleaned of all contamination he is permitted to install a pvc dike drain that is capped on both ends.
1616023362 2016-05-19 Production Wells No Violations On the day of my inspection I found that the dike was dry and free of water. None of the vegetation surrounding the dike showed any signs of stress. None of the vegetation bordering the lease road where Jeff spread the treated soil showed any signs of stress. There were no signs that the replacement tank was leaking. This violation is resolved.
1711827411 2016-03-21 Production Wells No Violations On the day of my inspection I found that dike had filled with water again. The 50 barrel tank was not leaking. Jeff Gress claimed that he has always had a problem with this dike filling with rain water and draining very slow. I did notice that there was a thin layer of ice on the surface of the water. I conducted a field test with a low range Quantab and found a chloride level of 417 parts per million.
