1220226070 2015-02-10 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with a pumping unit. The valves from the tubing and casing were closed. The polished rod was dry, the motor and gearbox were overgrown with weeds and the wellhead was blocked in with straw bails. I found no storage tanks. There was a separator near the wellhead. There was plastic gas line connected to the separator. No identification was posted. This well has had no production reported since 2009. This well must be plugged within 180 days of the issuance of this compliance notice.

1441721618 2015-09-08 Plug / Plug Back No Violations On the day of my inspection I found that Mike Wheeler was rigged up to plug the well. He was running 1.5 tubing to a depth of 3437. Cemenco pumped a 30 sack class A cement bottom plug.
1441814323 2015-09-09 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler ripped the 4.5 casing at 2933. He pulled the casing from the well and then ran 1.5 tubing back in to 2496 in order to set a cement plug at the top of the Big Lime.
1441897523 2015-09-10 Plug / Plug Back No Violations On the day of my inspection I found that Mike Wheeler had 1.5 tubing ran to 2496. Cemenco established circulation and pumped a 200 cement plug at the top of the Big Lime. Cemenco also pumped cement plugs through the Berea and at the base of the surface casing. Circulation was established and maintained through both plugs. The fluid started to fall when Cemenco was pumping the surface plug. Wheeler pulled the remaining tubing and they found the fluid to be down approximately 180 to 200. Cemenco bridged the well at the top of the fluid with block and stone, and filled the well to surface with cement. The cement stayed up in the surface casing 10 inches from the top.

1507789029 2015-08-21 Production Wells No Violations On the day of my inspection I found that the well was still sitting idle. The wellhead and pumping unit were surrounded with shrubs and weeds. The belts and pulleys on the motor were entwined with vines. Access to the wellhead was blocked by hay bails. There was a plugging permit issued on 3/3/15. I called Dwayne Dugan to ask when he planned to plug the well. He argued that he never received the compliance notice and that he voluntarily obtained the plugging permit, thereby giving him two years to plug the well. I informed him that we did send the compliance notice on 2/11/15 and it stipulated that the well was to be plugged within 180 days. Dwayne called me back after he located the original compliance notice and agreed on the previously discussed plug-by date. He asked for 30 days to get a rig lined up. I granted him a 30 day extension from today.
1991035303 2016-06-08 Final Restoration No Violations On the day of my inspection I found that the well site and access road had been graded out to the contour of the surrounding landscape and the were growing with well established hayfield vegetation. There were no erosion issues. There was still a vertical separator and some plastic line left along the fencerow where the tank battery was once located. I called Travis Dugan and left a message.
