0240384600 2018-09-17 Final Restoration No Violations On the day of my inspection I found that the well site and access road were covered in agricultural crops. There were no oilfield materials or debris left at the site and there were no signs that the plugged well was leaking. The tanks were still in place at the main driveway for the farm. The identification sign was blank, so I could not determine if they were being used for another well. I called John Mason, but he did not answer so I left a message. I found another well on the same farm, the Foster/Double D #2, permit 6983. The pumper showed up while I was conducting the inspection and confirmed that the tanks were in use for permit 6983. The final restoration for the Chopper #3 is complete.

1522849777 2018-04-04 Plug / Plug Back No Violations On the day of my inspection I found that Mason Drilling was rigged up to plug the well. The rods and tubing were still in the well. There was one 300 barrel frac tank and a pipe rack staged near the wellhead. I spoke with John Mason about setting a cement plug through the Utica/Point Pleasant zone. We discussed different options for setting the plug, and John said that he would get back to me once he had a plan.
1523302167 2018-04-09 Plug / Plug Back No Violations On the day of my inspection I found that Mason Drilling had pulled all of the rods out of the well. I spoke with John Mason and he said that they would be in tomorrow to attempt to circulate with the tubing thats already in the well. John plans to set the bottom plug with that tubing unless there is a problem with circulation. John also said that he has been in touch with DOGRM Deputy Chief Bob Worstall to discuss the proposed cement plug across the Utica shale, but they have not come to a decision yet.
1523558065 2018-04-12 Plug / Plug Back No Violations On the day of my inspection I found that Mason Drilling was circulating salt water with 2.375 inch diameter tubing set at a depth of 5384 feet. Mason has his own cement mixing and pumping apparatus and two pallets of Type ll, class A, Portland cement, totaling 31, 94 pound bags. I weighed their fresh water and found it to be at 8.4 pounds per gallon. Mason started mixing his cement and pumping it down the hole. I took several samples to monitor the weight. The cement averaged 13.5 pounds per gallon. The well was circulating and Mason had lift pressure that increased to 290 pounds when he was finished with his displacement of 19.5 barrels of salt water. I told John that his cement was light and he must devise a better method of getting it up to weight before he pumps any more plugs with that method. When I asked about monitoring the cement for the rest of the plugs John said that he was going to use a grout mix of Portland cement and sand, brought in by ready-mix trucks and dumped into his mixer. I conversed with inspector Pat Shreve about Johns proposal. Pat contacted Deputy Chiefs Dave Clause and Bob Worstall, who both denied the proposed use of grout for the rest of the plugs. I told John that he cannot use sand in his cement mix; he may use ready-mix trucks, but the slurry must be straight Portland cement. Masons crew pulled enough of the tubing to get above the cement top and shut down for the day.
1523985867 2018-04-17 Plug / Plug Back No Violations On the day of my inspection Mason Drilling brought Jet Wireline Services in to shoot the production casing. They ran a log on the well to ensure the casing was free where John Mason intended to shoot it. They also tagged the bottom plug at a depth of 4970 feet while they were logging the well. John Mason wanted to shoot the casing at or near 3450 feet and they found a collar at 3448 feet. Jet Wireline parted the 4.5 inch diameter casing at a depth of 3448 and Mason pulled one joint. John and I discussed the cement requirements for the remaining plugs. John gave me a copy of the proposed plugs with the cement volumes calculated to cubic yards, since he will be using ready-mix trucks to bring in the cement. John was insistent that he run approximately 30 percent sand in his cement blend, as nine-sack grout is an approved material for the plugging of oil and gas wells in the state of Ohio. I gave John a copy of Ohio Administrative Code 1501:9-11-07(B) which specifies the approved plugging materials and compressive strength requirements.
1524502803 2018-04-23 Plug / Plug Back No Violations On the day of my inspection Mason Drilling pulled 102 feet of the 4.5 inch diameter casing to bring it up to a depth of 3350 feet. Mason established circulation with salt water. Holmes Readymix arrived with 6.5 yards of Portland cement with 30 percent sand. I collected a sample of the cement and weighed it at 17.6 pounds per gallon. The cement truck driver added some more water and I weighed it again at 17.4 pounds per gallon. Mason said that he wanted to run it, so I took a sample and let him begin. They were able to re-establish circulation quickly and they pumped a 5 barrel fresh water spacer ahead of the cement. Mason started pumping the cement at 11:00. Masons pumps started having problems at 11:33 and the flow was stopped for approximately 10 minutes. They got the pumps back up and continued. The well continued to circulate throughout the time they were pumping. The triplex pump finally quit and John said that he was going to circulate all of the cement out of the well. We had the cement truck driver check his load and he said that he only had a half yard left on the truck. John went into his displacement with a 5 barrel fresh water spacer followed by 39 barrels of salt water. They maintained good circulation throughout the displacement process. Once the plug was down Mason pulled 18 joints of the 4.5 inch casing to get out of the cement and started breaking down the pumps. I told John that I need cement tickets for the sack cement and the ready-mix.
1524763041 2018-04-26 Plug / Plug Back No Violations On the day of my inspection I found that Mason Drilling had the 4.5 inch diameter casing pulled up to a depth of 2264 feet for the Big Lime plug. Formation Cementing established circulation and pumped a 200 foot cement plug. Mason pulled the casing up to 808 feet for the Berea plug. The top of the Berea plug was going to be above the bottom of the surface casing plug, so we decided to combine the two plugs. Formation Cementing established circulation and pumped a 350 foot cement plug. Mason pulled the casing up to 200 feet and checked the fluid level. They pulled the remaining casing and set a stone bridge at the top of the fluid at 150 feet. Formation filled the well from 150 feet to surface with cement. The cement stayed up at the top of the surface casing when they finished. John Mason said that he would be back tomorrow to cut the casing, cap it with a steel plate, and bury it.

2045080555 2018-04-11 Plug / Plug Back No Violations On the day of my inspection I found that Mason Drilling had a 300 barrel tank, two smaller tanks and a pump staged next to the wellhead. The tubing and casing valves were plumbed to the pump and tank. I spoke with John Mason who said that they circulated through the tubing into the tank to determine if they could use the existing tubing to set the bottom plug. He said the circulation test was successful and he planned to pump the bottom plug tomorrow. John reported that the tubing was set at a depth of 5384 feet. I spoke with Bob Worstall, who said that Mason was not required to set a plug across the Utica shale. Bob did say that he must set a thick enough plug from the Clinton sand to cover the Newburg dolomite. When I spoke with John he said that he has an open hole log from when the well was drilled and we will review it tomorrow to determine the appropriate plug depths.
