0065999332 2019-05-06 Preliminary Restoration No Violations On the day of my inspection I found that the drilling pit was still not completely closed. The steel casing that was used for the frac job was still in place. There were still four joints of casing laying on the ground at the well site. There were still piles of trash laying around the well site. There has been very minimal work done to complete the preliminary restoration at this site. I tried to call Dean Moran and he did not answer. I sent him a message to call me. Dean returned my call and said that it has been too wet. I told him that he has to do something at the site, regardless of the weather. He said that he would come out today. I told Dean that he has one week to show some progress. I will return next Monday to follow up.
0471533449 2018-07-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich Drilling was moving in and rigging up. The drilling pit was dug but not lined. I did not find any sharp rocks or tree roots in the bottom of the pit. There was 50 feet of new 11.75 inch diameter casing on site. Kenny predicted that he would be spudding the well tomorrow. I told him that he needs to get a copy of the drilling permit posted in the rig before he spuds. I also told them that they need to line the ditch from the dump line to the pit, as the pit is located approximately 30 feet downhill from the well site.
0471790666 2018-08-15 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 1900 feet. Kenny said that he has not been drilling for the past few days. He was getting some oil out of the Berea when he ran the bailer, but he said it was making more water than oil. The pit liner was in good shape and the fluid level was good. There were no leaks or spills around the drill site.
0533504098 2018-08-02 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 1150 feet. He was starting to pick up some oil from the Berea that was accumulating on the surface of the pit water. I noticed a 2 inch diameter hole in the pit liner that was covering the downhill portion of the dump chute. I brought it to Kennys attention and he said that they would get it re-lined. I tried to call Dean Moran but I got no answer. The liner in the pit was still in good shape and the fluid level was still well below the top of the pit wall. I sent a text message to Dean Moran requesting that he get a new section of pit liner put down below the dump chute today. He responded that he would get it taken care of.
0534116059 2018-11-27 Preliminary Restoration No Violations On the day of my inspection I found that there were no changes at the well site since my last inspection. The drilling pit was partially closed and I called Dean Moran to ask when he was going to get it finished. Dean said that he has been trying, but the site is so muddy that he cant make any progress. His dozer and skid loader were still parked at the site and the entire area was extremely muddy. I asked Dean to keep an eye on the pit with regards to rain water. He said that he planned to get it finished as soon as the ground freezes up a little.
0577951490 2019-01-18 Preliminary Restoration No Violations On the day of my inspection I found that the drilling pit was still not closed. The ground was snow covered and I did not find any signs in the snow that any of the remaining fluid has been leaking, but it still poses a risk being left open all winter. I contacted Dean Moran and told him that he cannot leave it open until summer. He has to figure something out to get it closed as soon as possible. The pit closure was due on 1/16/19. I also told Dean that he needs to get his identification posted on the tanks.

He said that it had went on vacuum and the fluid was down approximately 700 feet. Kenny said that they were waiting on tubing and rods, and they were going to equip the well for a pumping unit before they started to rig down and move out. The fluid level in the pit was good and I did not see any holes or sags in the pit liner.

1211456661 2019-02-19 Preliminary Restoration No Violations On the day of my inspection I found that the dike had been built up around the tanks and the identification sign had been posted. The steel lines that had been laid for the frack job were still in place. A house gas line had been plumbed to the house on the property. There were still four joints of casing, trash and debris scattered around the well site. The drilling pit was still not completely closed. The ends had been pushed in and the center of the pit was mostly mud. There was a 2 inch layer of ice on top of the mud. I contacted Dean Moran, who claimed he was in the process of getting it finished.
1314377862 2018-08-20 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 2180 feet. He said that he was in the top of the Big Lime. They are waiting until they get through the Oriskany sand and assess the water production before they decide to run 7 inch diameter casing. There were no holes or sags in the pit liner and the fluid level was still 12 to 15 inches below the top of the pit wall. There were no leaks or spills around the drill site.
1473852916 2018-07-27 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 1055 feet. Kenny said that he would be at 1100 feet by the end of the day. He also said that he would not be drilling again until Wednesday next week. They had a section of sheet metal set up beside the dump chute that was deflecting the splashing back into the chute when Kenny dumped the bailer. There were no visible holes in the pit liner and the fluid was still 24 inches below the top of the pit wall.
1531489385 2018-07-13 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling the surface hole at a depth of 80 feet. He reported that he spudded the well on 7/11/18, and he got the 11.75 inch diameter conductor casing driven to a depth of 40 feet. The drilling permit was in the rig. There were no sags or holes in the pit liner, and the dump line ditch was securely lined. There were no leaks or spills around the drill site. Kenny said that the used 8.625 inch diameter surface casing was being pressure tested.
1531921607 2018-07-18 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling the surface hole at a depth of 390 feet. There was a string of 8.625 inch casing on the pipe racks. Kenny predicted that he would be running the surface casing tomorrow afternoon. I spoke with Kenny and I called Dean Moran to make sure they know the casing must be centralized. I also reminded Dean that he will need to have a bond log in the event that they cant establish circulation during the cement job. There were no holes or sags in the pit liner and the fluid level was well below the top of the pit walls.
1532356628 2018-07-23 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 585 feet. He had a copy of the bond log that was ran on 7/20/18 which showed the cement top at a depth of 76 feet. I called Dean Moran and he said that he was on his way to buy sack cement to top off the annulus. The pit liner was in good shape and the fluid level was not close to the top. There were no leaks or spills around the drill site.
1532614237 2018-07-26 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 870 feet. Kenny said that he used 18, 80 pound bags of sack cement to top off the annulus of the surface casing. There were no sags or visible holes in the pit liner, and the fluid level was well below the top of the pit walls. I did find that there was some residual splatter when they were dumping the bailer that was starting to accumulate on the ground beside the dump chute. They were just starting to get into the Berea sand and there appeared to be a little oil in the fluid that was bailed from the hole. I brought it to their attention and Kenny Jr, tried to use a front-end loader to build up a dirt mound on the side of the dump chute to deflect the overspray when they dumped the bailer. I called Dean Moran to discuss the situation and come up with a remedy. Dean said that he had some sheet metal that he would bring out to create a shield to deflect the overspray back into the lined dump chute.
1533132276 2018-08-01 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was not drilling. There was no one working at the drill site. The tools were suspended in the hole. The depth chart showed that they were at 1100 feet. There were no sags or holes in the pit liner and the fluid level was more than two feet from the top. There were no leaks or spills around the drilling rig.

There were no holes or sags in the pit liner and the fluid level was at least two feet below the top of the pit wall. The permit was in the rig, and I found no leaks or spills around the drill site.
1533823680 2018-08-09 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was not drilling. He was shut down for maintenance and an oil change on the drilling rig. The depth chart showed that they were at 1600 feet. There were no sags or holes in the pit liner and the fluid level was still well below the top of the pit walls.
1535035365 2018-08-23 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 2300 feet. Kenny said that he just reduced the hole from 8 inches to 6.5 inches in diameter. Kenny said that they picked up a little gas in the Oriskany, but no significant fluid. They still plan to run a 7 inch working string through the Berea and black grit. I did not find any holes or sags in the pit liner and the fluid level was 10 to 12 inches below the top of the pit walls. Kenny said that they would be calling Dean Moran today to have a load taken out of the pit. There were no leaks or spills around the drill site.
1535463945 2018-08-28 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 2628 feet. I found a two inch hole in t he pit liner approximately 12 inches below the top of the pit wall, and three inches above the top of the current fluid level. I called Dean Moran to discuss the issue. He did not answer and I could not leave a voice message. I took a photo and texted it to Dean, letting him know that he must take all precautions to keep the fluid level below the hole.
1536164514 2018-09-05 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was still not back to drilling. He was at the site and said that he was waiting on parts for the drilling rig. Dean Moran was also at the site. He had a load of fluid taken out of the pit and the patch was exposed. It appeared to be secure. Kenny said that his current depth was 2870 feet.

Kenny said that he was making close to 30 feet per day, and he was drilling to a depth of approximately 3370 feet. He said that he expected to be running casing by the end of next week. There were no holes or sags in the pit liner and the fluid level was 12 to 15 inches from the top of the pit wall.
1538662253 2018-10-04 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 3339 feet. Kenny said that he just got through the Packer Shell. He said that they had Cemenco scheduled for 3:00 tomorrow to cement the 4.5 inch diameter production casing. There were no visible holes or sags in the pit liner, and the fluid level was 8 to 12 inches below the top of the pit wall. They did not have the casing test paperwork for the used casing. I contacted Dean Moran to make arrangements to have it brought out to the drill site.
1538749180 2018-10-05 Casing String Witnessed No Violations On the day of my inspection I found that Kenny Rich was running the 4.5 inch diameter production casing. I received a copy of the casing pressure test paperwork. I called Dean Moran to request that he have a load of water taken out of the pit because the fluid level was getting close to the edge. I returned to the site three hours later and found that there was a water truck at the drill site taking a load out of the pit. Kenny drilled to a depth of 3371, and they were running the 4.5 inch diameter casing. The casing was equipped with 6 centralizers and a latch-down shoe. The cement company did not show up at the scheduled time. I waited until I was out of regular hours and left.
1539267918 2018-10-11 Drill / Deepen / Reopen No Violations Upon arrival on location, I witnessed Rich Drilling was drilling at the depth of 3421 feet. The pits were in good shape and the fluid level was 10 to 12 inches below the top surface. The location around the rig was clean and free of any spills.
1539615946 2018-10-15 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 3443 feet. They were through the Clinton sand, and Kenny said that they had about 5 to 10 more feet of rat-hole to drill. The frack job is scheduled for tomorrow morning. The pit liner was in good shape and the fluid level was good. I received a copy of the cement ticket from the production string cement job that took place on 10/5/18. Kenny said that the job went well. The ticket showed that Cemenco pumped 130 sacks of class A cement.
1539703756 2018-10-16 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Dean Moran had 4.5 inch steel casing ran down the hill from the drill site for the frack job. He also has his frack tanks in place. Producers has not shown up yet. I spoke to Dean and let him know that I had to leave the site for a plug job approximately 10 miles away. I told Dean to call me if there were any problems. When I returned I found that Producers was just finishing up with the job. I met with Dean to look at the frack report. The Clinton broke at 819 psi. and the well treated at an average pressure of around 1400 psi. The initial shut in and 5 minute pressures were around 915 psi. There were no significant pressure changes throughout the job, and there were no leaks or spills. Dean said that he was not going to swab the well or flow back. He was going to leave the well shut in until it went on vacuum and then equip it to be pumped.
1540304984 2018-10-23 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was rigging down and moving out. The well was equipped with a wellhead, tubing and rods. Kenny said that they should have the pumping unit installed tomorrow. There were no sags in the pit liner and the fluid level was down 12 inches from the top of the pit wall.
1540824990 2018-10-29 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich and Den Moran were at the well site pumping the well. They had just got a natural gas powered pumping unit installed and operating. Dean said the well started making gas right away and it was up to 125 pounds. He also said that it was starting to make oil. Dean had three 100 barrel steel tanks and a vertical separator in place, and he already had his flow line buried from the well down to the tanks. The earthen dike was not complete yet. The drilling pit was still open. Dean said that he would have it drained in the next two days and start closing the pit. Kenny Rich had most of his drilling rig equipment moved down off of the hill and staged for transport.
1541446310 2018-11-05 Preliminary Restoration No Violations On the day of my inspection I found that Dean Moran was working at the well site laying his gas sales line. The well was pumping and the three 100 barrel steel tanks were diked. The drilling pit was still open and it was full. It was not full enough to run over, but it was within two or three inches. I told Dean that he needs to get a load pulled out of the pit right away, before we get more rain tomorrow. He said that he would get someone out here today or first thing in the morning. The main route, SR 60, into Layland is flooded in both directions from CR 19. A water truck can still access the site from the west on CR 19. I discussed this with Dean and made sure he was aware that he needs to get the pit fluid level drawn down right away.
1541696020 2018-11-08 Preliminary Restoration No Violations On the day of my inspection I found that the fluid level in the pit had been drawn down. Dean Moran was at the site when I arrived. He said that they took two loads out the next day after my last inspection. There were no holes or sags in the pit liner. Dean was at the site pumping the well. He said that he can start closing the pit as soon as the ground dries out a little. Kenny Rich was also at the site removing the last of his drilling equipment.
1557931677 2019-05-15 Preliminary Restoration No Violations On the day of my inspection I found that all of the materials and debris that were left around the wellsite after drilling and completion had been removed. All of the trash had been removed. The drilling pit had also been completely closed. The site still needs to be graded out and seeded for vegetative cover. I contacted Dean Moran about his plan to complete the restoration. He said the landowner was going to finish grade the site to his own liking, and then they would get some grass seed down.
1856285184 2018-07-11 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was still rigging up to start drilling. The pit was lined and they had liner ran from the dump line to the pit. Dean Moran showed up and I spoke to him about getting a copy of the drilling permit posted in the rig before they started drilling. Dean and I also discussed the requirements for his casing program, regarding used casing and centralizers.
1885628755 2018-07-19 Casing String Witnessed No Violations On the day of my inspection I found that Kenny Rich Drilling had used, tested 8.625 inch diameter casing set at a depth of 482 feet. Kenny reported that the casing was equipped with two centralizers and a guide shoe. Cemenco pumped gel and cotton seed hulls ahead and went into the cement. They pumped 120 sacks of class A cement and displaced with 30 barrels of fresh water. Cemenco landed the plug and pressured up to 500 pounds for 5 minutes. The well never circulated throughout the job. I told Dean Moran that he must have a bond log run on the surface job before they could go back to drilling.
2091637416 2018-09-18 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kenny Rich was drilling at a depth of 3030 feet. There were no holes or sags in the pit liner, but the fluid level was getting high. I contacted Dean Moran to request that he have a load taken out of the pit. He said he would get a water truck out as soon as possible.

The drilling tools were in the hole and there were materials scattered throughout the drilling rig. There was a line clamp on the drilling line and it was chained to the elevators. I tried to call Dean Moran but he did not answer his phone. The fluid level in the pit was covering the area where I found the hole last week. I could not tell if the hole had been repaired of not. Dean said that he was going to patch it the last time that we spoke.
