| 0066052266 | 2017-11-27 | Production Wells | No Violations | Inspector Pat Shreve found this well to be in a state of abandonment in a forested area to the east of the township road. There were no signs of an access road to the well and there was no production equipment found in the area. The well was equipped with a 4.5 inch wellhead, tubing and rods. All of the lines were disconnected and open to the atmosphere. There were no leaks or spills around the wellhead. Pat cleared the brush from around the wellhead and marked it with yellow flagging. |

| 1607430248 | 2019-04-16 | Production Wells | No Violations | On the day of my inspection I was accompanied to the well site by SE Regional Supervisor Brent Bear. We found that the well was equipped with production casing, tubing and rods. The surface casing was open to the atmosphere. There were no flow lines connected at the wellhead. We found no signs of fluid leaks from the well, and we did not detect the odor of leaking natural gas. There was no conspicuous access to the wellhead and there was no production equipment associated with the well. There was no well identification posted at the wellhead. The only production reports ever reported for this well are from 2010 to 2017, except for 2011, 2012 and 2015, which were omitted. All the reported years show zero oil or gas production. There were multiple sections of plastic and steel line scattered through the forest around the wellhead. I took updated photographs of the wellhead. |
