0086419327 2017-05-18 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I noted the following: a wellhead with pump jack system and associated tank battery, located in an otherwise wooded and remote portion of the township, on Cady Road. Access to the wellhead is afforded by a gravel-paved access road that has become partially overgrown along its path but is passable. Fittings and connections at the well and battery were secure. The load line was plugged and locked. The secondary containment was intact and dry with no signs of significant leaks, spills, or problems. I noted that the ID signage had become faded and advised the owner about the need for maintenance at the well.
0828307177 2015-05-07 UrbanProduction Wells No Violations I conducted a status check on this well with DOGRM Inspector - John Fleming and District Manager Rick Anderson. The wellhead is equipped with a pumpjack and the valves on the wellhead are open. The belts for the pumpjack were on the ground next to it, but the electric to the wellhead was turned on. The tank battery consists of 2-100 BBL production tanks, mechanical separator and a 1500 gallon steel brine tank. The tank valves on the production tanks have bull plugs but no locks. The tank battery was diked, but the northwest corner of the dike was low and needed to be built back up. There was a domestic line with a meter. The well is not properly identified with a clear, legibly marked well identification sign. The lease road is stable and drivable. Contacted Dallas White, the contract pumper for Twin Oaks and discussed the items that needed addressed. Will follow-up and reinspect the site by 5-20-15.

Unit spacing is in accordance with the regulations. There is no id at the gate. I observed a significant amount of oil staining and oil impacted material around the production tank and the mechanical separator. I contacted Twinoaks Oil and Gas about the need for proper signage at the gate and the need for mitigation of oil release at the tank battery.

At the time of this inspection, I observed the following elements: a electric pump jack system and tank battery with two 100-barrel steel production tanks and one 25-barrel steel brine tank. Fittings and connections at the wellhead and battery were plugged and secure. The secondary containment was present and intact. I noted that the identification signage at the battery was faded and hard to read and I notified the owner.

Production reports were current and up-to-date. The dedicated access road was in good condition. I walked and inspected the areas surrounding the wellhead and battery and found the natural greenscape to be free from signs of impact or degradation.
