The earlier issue regarding ownership has been resolved. Fittings and connections at the wellhead are secure and free from leakage. Signage at the well needs updated. The battery is overgrown. The shed-like structure which houses the separator is unsecure. I have notified Jim Gessel previously about the maintenance issues. I spoke with the Green Energy well tender (Jim). He indicated that maintenance issues were going to be taken care of ASAP. |

The signage at the well is in poor condition. The battery is surrounded by very dense vegetation and is not being properly maintained. The vegetation over-growth needs to be removed. There is no secondary containment. The listed well owner does not agree with production reports. I spoke with Jim Gessel of Green Energy. He indicated that Green Energy is the owner/operator, and not Miller A Oil Co. Im following up with ODNR Fiscal and Bonding to see what is required to correct this. I also advised Green Energy that the ID sign needs repaired/replaced and the tank battery system needs secondary containment if it is to be utilized. Otherwise the system should be removed. Jim advised that he is checking to see what the strategy for repairing or removing the battery will be. |

At the time of this inspection, the well and battery were located on an otherwise undeveloped parcel. The surrounding area is comprised of a newer single-family residential development. The well was comprised of a 4.5-inch casing riser with reduction swage valve and flow line connected to a tank battery system comprised of 50-barrel steel tank and separator. The separator is housed in a tin-wall shed with the tank adjacent to the shed on the south side. There was a makeshift door to the shed which was unsecure. The tank appears unused. ID signage at the wellhead which had been noted to be missing at an earlier inspection has been replaced, but warning signage is missing at the battery. I contacted Jim Gessel with Green Energy and advised him that the tank, and any other unused elements must be removed from the location, the enclosure door at the battery, must be locked and secure, and warning signage must be displayed. We agreed to meet at the location on 01/06/2016 to review the maintenance issues. |

During the previous inspection, I had noted the following maintenance deficiencies: the shed which houses the separator was not secure and was lacking signage. The 50-barrel steel tank appeared to be unused, but after a site meeting, we agreed that the tank could remain in place, but would need an access door from the outside or from within the shed. At the time of this inspection, the exterior shed door was found to be secure and signage was updated. There was still no access door to the tank and I notified Jim Gessel that a door was required. Otherwise, fittings and connections at the well were secure and free from leakage. The well enclosure was locked and secure. |

I had notified Jim Gessel of Green Energy about the ruts. We agreed that they would fill in the ruts with topsoil and then grade them and re-seed in the spring. At the time of this inspection, the ruts have been filled and graded. No further action is required until re-seeding in the spring. |

At the time of the previous inspection, I had noted the following maintenance issues: the tin-walled enclosure which houses the mechanical separator was open and unsecure, warning signage was lacking, and the existing 50 BBL steel tank appeared to be unused. At the time of this inspection, I met with Jim and Steve Gesssel of Green Energy Inc. at the well location. The enclosure has been secured and locked. ID and warning signage has been updated. According to Steve Gessel, the well does not commonly produce fluids but the tank is necessary to collect fluids in the event of production. I advised that the tank enclosure must have an entry door, which was lacking. They agreed to remedy the entry door. Otherwise, fittings and connections at the wellhead and battery were secure. The wellhead enclosure gate was locked and secure. I did not observe any other issues at the time of this inspection. |

Elements of the well consist of a production wellhead with standard valve assembly connected to subsurface flow lines. The wellhead is located in a closed and locked chain-link enclosure. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located south of the wellhead in an area completely overgrown with vegetation. The battery is comprised of 1 steel 50-barrel steel tank, and a horizontal separator. The separator is located in a corrugated metal-sided shed and the tank is located outside the shed in a fenced enclosure. The battery appears to be unmaintained and not in use. There is an ID placard at the wellhead, but its in very poor condition and needs replaced. The wellhead and battery are located in a topographically level area . Surface drainage from the producing unit appears to be to the west, towards lower lying areas. Access to the wellhead and battery are from the road to the north but there is currently no defined lease road. The area of the well is a vacant lot which is overgrown with vegetation. I will contact the well owner about replacing the ID placard and maintenance/repair to the other production appurtenances. There are otherwise no issues or problems requiring attention at this time. |
