0542491476 | 2016-12-19 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following elements: a wellhead with mechanical plunger-lift system in a small, chain-link enclosure. The well was located at the edge of an asphalt-paved parking area and on the fringe of a ball field. The enclosure was locked and secure. The interior of the well containment was dry and free of vegetation, and evidence of leakage or other problems. Fittings and connections were secure. The associated battery is located 500 feet north of the wellhead, at the fringe of another ball field. The battery was comprised of one 100-barrel steel production tank, one 50-barrel plastic brine tank. Load lines were locked and secure. Enclosure gates were locked. Tank lids were closed and secure. Secondary containment was intact. I walked the areas around the wellhead and battery. These areas consist mainly of managed turf grass and were free from signs of drainage problems, or other signs of impact relating to the on-going production. |
0761393691 | 2013-11-25 | UrbanProduction Wells | Violations Noted | Safety equipment and locks have been remedied and no other problems remain. |
1064584725 | 2003-09-25 | Production Wells | No Violations | Inspected well site and tank battery. 210BBL seperator plasitc brine tank. |
1559252469 | 2016-09-09 | UrbanProduction Wells | No Violations | I inspected this well location as part of on-going work-over activity intended to replace the plunger lift production system with a pumping unit. At the time of this inspection, the plunger lift system has been removed and roads and tubing were installed pending placement of the pumping unit. I noted that there was some gas leakage around the casing head packing, and I called and notified Jim Gessel with Green Energy, who provides contract well service for this unit. |
1735268112 | 2013-07-24 | UrbanProduction Wells | Violations Noted | The producing unit consists of a wellhead with mechanical plunger-lift unit and above-ground storage tank (AST) battery. The wellhead is located in a chain-link enclosure which is locked. The well is currently producing. There is ID at the wellhead. I did not observe any signs of leakage or problems. The AST battery is comprised of one 140-barrel production tank, one 50-barrel brine tank, and mechanical separator in a chain-link enclosure. There is a unit ID at the wellhead. Secondary containment is good. The unit is lacking safety equipment in accordance with 1501:9-9-05E. I advised Green Energy that the safety gear should be addressed before the start of school. |
1789244959 | 2018-07-13 | UrbanProduction Wells | No Violations | I inspected this producing well as part of routine inspection activity. The battery and wellhead were in separate chain-link enclosures which were locked and secure. The former plunger-lift system had been replaced with a pump jack. The well was in production. The fittings and connections were secure. The load-lines were locked. Secondary containment at the battery was present and intact and dry. |
1968404325 | 2016-08-29 | Work Over Reconditioning | No Violations | I inspected this well location pursuant to on-going work-over being conducted by Green Energy for Gilmour Academy. The well contractor was Hagen Well Service. At the time of this inspection, crews were preparing to remove the wellhead which had been permanently welded to the 4.5-inch casing following a previous incident. The casing head was being removed by grinder saw. Crews had previously blown down the well and had assembled and flexible line to allow gas to vent away from the well during grinding. The crews had stationed a fire watch person with fire extinguisher during this activity. The wellhead was successfully removed and the crews transitioned to begin removing production tubing. Following the removal of tubing, the crews will install pumping rods and a low-profile pumping unit. I met with both Jim and Gessel of Green Energy and a member of the Gilmour Academy maintenance staff to disuss planned activities. |
