1192494645 2014-06-10 Final Restoration No Violations I inspected this former well as part of the on-going evaluation and assessment associated with post-plugging management. The well was originally completed in 03/1986 and plugged and abandoned in 06/2002 by the owner/operator. In 04/2014, the current landowner contacted ODNR regarding potential issues pertaining to construction on or near former wells. At that time, the well was inspected and found to be leaking a small amount of gas. ODNR recommended the well be drilled out and re-plugged or at a minimum, a vault and vent system be constructed to protect the former wellhead and provide passive venting. It was also recommended that no structure be built on the well, and at least 10 feet or more of clearance be maintained, in perpetuity, around the location. At the time of this inspection, site work for the future home is on-going. Gonzoil has been contracted to build a vault/vent system, which is now in place. The system is comprised of a cylindrical, pre-cast concrete utility-type vault with a steel, bolt-down man lid, and side mounted passive vent system constructed with black, plastic 2-inch diameter pipe which will extend to a remote location elsewhere on the property.
