0478085245 | 2017-10-19 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity. The well and battery are located on a parcel utilized for materials storage. At the time of this inspection, I observed the following: a wellhead with pump jack system which was in operation. Fittings and connections at the pump jack were secure and free from signs of leakage or problems. The chain-link enclosure was locked and secure. The associated tank battery is located in a separate chain-link enclosure which was locked and secure. The battery is shared with the #2 and #3 units. The fittings and connections were secure. The secondary containment was intact and dry. Signage was present. I walked the areas surrounding the well and battery and found no signs of degradation or impact to the natural greenscape. |

0828457264 | 2017-02-10 | UrbanProduction Wells | No Violations | I inspected this producing well as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery located on a large parcel utilized for commercial sand and gravel storage along the east bank of the Cuyahoga River. Fittings at the wellhead including flow-line connections and fittings were secure and free from leakage. The tank battery was comprised of two 210-barrel steel production tanks, and one 100-barrel steel brine tank with mechanical separator. The battery is shared with the #2 and the #3 wells (#2-1801 and #2-1802). The secondary containment was present and intact. The visible system connections were secure and free from leakage. Load lines were plugged and secure. The well is surrounded by gravel drive and materials storage operations. The battery is bordered on the north by a drainage channel to the Cuyahoga River. The battery is bordered on the remaining sides by flat-lying areas of gravel drive and materials storage operations. I walked the areas surrounding both the wellhead and battery with a focus on the down-gradient area. I did not observe any signs of erosional deficits or impacts associated with the producing unit. The access road was mostly well drained and stable with no signs of wash-out or other drainage problems. |
0889012677 | 2014-05-15 | UrbanPreliminary Restoration | No Violations | I inspected the producing unit which was completed in 2004. The unit is comprised of a wellhead with valve tree and pump jack system in a locked chain-link enclosure. There is signage at the wellhead. The associated aboveground storage tank (AST) battery is comprised of 2 210-barrel steel production tanks, 1 100-barrel brine tank, 2 vertical separators, and associated valving and appurtenances. The battery is enclosed in a locked chain-link fence. The load-outs are bull-plugged. There is signage. The secondary containment is well maintained. The interior of the battery is free of liquids, staining or other signs of spills, leakage or other problems. The unit spacing is proper and in accordance with rules. The wellhead and battery are located on the east bank of the Cuyahoga River, in an industrial materials storage yard. There is no specific or defined lease or access road. A majority of the property is covered with gravel and loose slag material, with the balance comprised of miscellaneous vegetation. Drilling pits and other disturbance related to the drilling activity have been restored. Drainage is generally via surface flow to the west and the bordering Cuyahoga River. Owing to the loose granular nature of the substrate, erosional impacts from surface drainage are minimal and no specific engineering management appears warranted. Preliminary Restoration is complete. |
1178358407 | 2018-08-22 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The well was in active production. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery and found no signs of problems. I noted that the ID signage at the wellhead had become faded and I notified Duck Creek about the need for maintenance. |
