Plugging and abandonment was completed on 8/21/13, under witness by an ODNR inspector, and a report filed. At the time of this inspection, the areas of the former location of the wellhead and battery had been re-graded and re-seeded with vegetation established. Final Restoration is passed.

1362068022 2013-02-25 UrbanProduction Wells No Violations The producing unit consists of the following elements: A production wellhead and tank battery. The production wellhead consists of a pumpjack system in a 6 enclosure. The tank battery is comprised of 1 100-barrel (BBL) tank, 1 50-BBL tank, mechanical separator, and associated piping and appurtenances enclosed in an 8 enclosure. The secondary containment is good, except that there is a dike drain which was open at the time of my visit. I spoke with Ralph Gregory of Bass Energy. I advised him that the following actions are required: the open dike drain must be closed. He indicated that he was heading to the site the next day to address the issues.

The wellhead was formerly located in theback yard area of a private resicence on County Line Road. The wellhead is now sealed and covered with fill.
