The well and battery are located in the grassy and wooded median of Gates Mills Blvd., with suburban residential beyond. At the time of this inspection I noted the following: a well with plunger-lift system and associated tank battery in separate board-on-board enclosures which were locked and secure. The battery is shared with the GMV 4D (2-2044). The load-line valves were plugged, locked and secure. The secondary containment was intact. The production reports were current. The well was producing with an active chart record. I walked and inspected the areas surrounding the well and battery and found no indications of problems.

At the time of this inspection I observed the following: A wellhead with plunger-lift system and associated tank battery (shared with the #2-2044 well). Fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems. The tank lids were closed and sealed. Load-out lines were secure with locks. Secondary containment was intact. The area surrounding the wellhead and battery was comprised of woods and meadowland. I found no signs of problems or impact associated with the on-going well production.

Spacing is good. Thief hatch on tank is closed and diapers have been placed in the thief hatch. There is oil on the ground in the tank battery. Operator has a few diapers on the ground to soak up the oil. Leaf litter in the tank battery, that has oil on it will need to be removed. Tank battery needs spilled oil cleaned out of it. Permit ID signs are up at the entrance and at the battery. Two leaks at the well head, minor. Two fittings are leaking oil is not making it to the ground. Fittings will need tightened. Vegetation around site is established. Landscaping meets HB-278. Arborvitae has been damaged by deer browsing.

Fittings and connections at the wellhead are secure and free from signs of leakage or problems. The wellhead enclosure area is well maintained. The 8.625-inch casing is valved and open to atmosphere. Areas surrounding the wellhead and battery are comprised of tall meadow grasses and mixed deciduous and pine forest. I did not note any signs or evidence of impact relating to the production activity.
