The requirements for Final Restoration have been met.

At the time of this inspection, activities associated with mechanical ripping of the 4.5-inch production casing were underway. 1431543711 2015-05-06 UrbanProduction Wells Violations Noted I was on site to perform an inspection with DOGRM Inspector Shannon Mullins. The tank battery and wellhead are enclosed with a board on board fence. Upon our inspection we found that the tank hatches on the one 140 barrel tank is equipped with the proper sealed hatches and lightning arrestors and a charcoal filter. There were no locks on the water or oil pick up lines. The tank battery entrance gate was locked. The wellhead gate was locked. The well is equipped with a pumping unit and an electric motor. All unused chemical pumps and drums needs to be removed. The electric boxes were not locked. ID signs are posted but are not in compliance with 1501:9-9-05--( E). 
 1) Add additional identification on sign as required by 1501 :9-9-05  ( E )
 2) Install locks on oil and water valves at tanks or load lines and electric box.                                               
 3) Check water inside dike for chlorides. If below 500ppm chlorides pump water from dike.

At the time of this inspection, the well was blown down, and a bottom (Clinton Formation) plug was bullheaded through the 4.5-inch production casing without problem or incident. Attempts to mechanically rip the 4.5-inch casing commenced and were not completed by the conclusion of the work day, and would be resumed the following day. Details of these activities can be found in the associated plugging and abandonment report.

I did not observe any problems at this time of this inspection.

Fittings and connections at the wellhead and battery are secure and free from signs of leakage or problems. Load-outs are secure. Gates are locked. Process safety equipment is present. The access road is good, passable condition with no washouts or other erosional deficits or signs of impact.
