At the time of this inspection I observed the following: a wellhead with plunger lift system and associated tank battery in separate board-on-board enclosures which were locked and secure. The valves and connections at both the well and battery were secure and free from signs of leakage. Process safety equipment was present. ID signage was present and accurate. The non-compliant tank lid that I had previously observed had been replaced with a sealing lid as required. The areas surrounding the well and battery were comprised of woods and turf grass lawn. I walked that areas around the wellhead and battery and found no signs of impacts or problems. |

At the time of this inspection, I observed the following: Ingress and egress from the well area is afforded by the shared residential driveway. A wellhead with mechanical plunger lift system is located in a board-on-board enclosure in the back yard area, past the house. The well enclosure is locked and secure. Fittings and connections at the wellhead are secure and free from leakage. The 8.625-inch casing annulus was exposed and plumbed into the system. Areas around the wellhead consist of turf grass which was free from signs of stress or impact. The tank battery is located 75 south of the wellhead in a wooded area which is accessed from the parking lot of the adjacent church. The battery is comprised of one 100-barrel, low-boy steel production tank, one 50-barrel steel brine tank. At the time of this inspection, the access gate to the battery is locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. Tank lids were closed. The load-outs were plugged and locked. The battery is surrounded by mature evergreen trees. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation. I did observe the following maintenance issue: the production tank lid is an old style flap lid which is not compliant and must be upgraded or replaced. I met with the new owner and his well tender at this location and discussed the need for the tank lid freplacement. |

I advised the landowner that in the spring, we could make a better assessment. |

Reportedly, the mounding has grown increasingly notciable. I observed the mounded area to be more or less coincident with the former location of the drilling pits. The area is mounded up 12 to 18 inches above the surrounding grade, with pronounced, linear expansion cracks along the top. I contacted John Vizner with Seagull Development and he indicated that they are aware of the problem and are working to resolve it. It is belived to be a result of the extreme winter cold, and resultant frost heave. |

Corp. have re-graded and seeded the area and restored the location. The producing unit is functioning normally. I did not observe any signs of leakage or problems at the wellhead or tank battery. |
