1139728415 2015-12-16 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The well and associated battery are located in a clearing in an otherwise wooded portion of a residential lot. The cleared portion of the lot is generally flat-lying. At the time of this inspection, connections and fittings at the battery and wellhead were secure and free from signs of leakage or problems. The individual enclosure gates were locked and secure. Load lines were bull-plugged and locked. Tank hatch lids were closed but one tank was missing a closure wing nut and rendered not sealed as required. The electrical box was locked. Carbon filter, low-pressure relief valve, and spark arrestor elements were present. Secondary containment at the battery was intact and the interior is dry. I walked the surrounding areas and did not observe any signs of impact. The short access road was level, well drained, and maintained. I notified Ralph Gregory with Bass Energy regarding the missing wing nut. He indicated the deficiency would be rectified immediately.
1507642191 2017-10-10 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead plunger lift and associated tank battery located on a wooded parcel along Mayfield Road. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery, and found no signs of impact or degradation attributable to the production system. The egress and ingress road was in good condition.
1608721583 2011-12-28 UrbanPreliminary Restoration No Violations On 12/28/2011, DNR Inspector Richard Skidmore completed an urban preliminary restoration well inspection for DNR Inspector Ray Wallace. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
