There is an unpaved access road which travels east from Broadview Road. At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The tank battery is comprised of a total of two 100-barrel steel production tanks, one 50-barrel poly brine tank, and mechanical separator. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. Evergreen screening is required at the battery and wellhead. There is some remnant silt fence which should be maintained or removed if no longer needed. The entry point requires a locked, gated entry with signage. The 8.625-inch casing is not exposed at the surface to facilitate monitoring as required. Signage is present but lacking complete information. There is gas leakage at the back-pressure (Baird) valve. An empty 55-gallon drum and chemical pump should be removed if not used. Carbon filters must be periodically evaluated for proper operation. In addition, the following safety issues were noted. Load lines are not locked. Brine is actively leaking from the 2-inch flange at the brine tank. Lightning arrestors are missing from the production tanks. These safety issues must be remedied as soon as practicable or by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding this Compliance Notice. |

Wellhead is in good operating condition and consists of a pump jack style unit. There is no evidence of any leaks, spills or poor conditions. The tank battery consists of two (2) 100 Barrel lowboy tanks with vents and a laser meter to read oil and gas levels in the tanks. All caps and plugs are secure and there are no locks on the load lines, however they are behind a locked gate and are not accessible to the public. There is one 50 Barrel brine tank and one separator in the unit. Fence is board on board and the dike around the tank battery is capable of containing fluids to the standards in 1501:9-9-05. Preliminary restoration passed. |

Prior to my arrival, crews from the owner/operator had dismantled the pumping unit and removed the sucker rods and production tubing. Residual pressure had been relieved and the well was open to atmosphere and stable with no appreciable gas being produced. Plugging and abandonment activity were executed in compliance with OAC 1501:9-11. Plugging activity is detailed in the associated plugging report. Upon establishment of the top surface plug, the well was noted to be stable with no gas production. A steel plate with well ID had been prepared and will be secured to the wellhead prior to burial at 30 inches below grade as required. Final restoration activities will be on-going until completion as required. Discussions with the well owner indicate some elements of the access road and the existing sales line may be left in place pending landowner waiver. |
