Production tank lids were not latched down as required. The dike drain riser was not secure and leaking. There was a significant gap between the bottom of the fence enclosure and the ground surface at the well enclosure and battery rendering the enclosures unsecure. Signage was lacking complete information. There was a notable amount of grease leaking down from the Pitman bearing on the pump jack which needs addressed. The 8.625-inch casing was not exposed for monitoring as required. In addition, the following safety issues were noted. Load lines were not locked. Brine was actively leaking from the 2-inch flange at the brine tank. The owner had been notified of the deficiencies. At the time of this inspection, the listed violations and deficiencies had been remedied. Tank lids were closed and secure. Load-lines were locked. Signage had been updated. The surface casing nipple was exposed with pressure gauge. The leaking grease had been remediated and the brine leak repaired. |

The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The shared tank battery is comprised of a total of 6 140-barrel steel production tanks, 3 50-barrel poly brine tanks, and 3 mechanical separators. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. The dike drain riser is not secure and leaking which requires remedy. There is a significant gap between the bottom of the fence enclosure and the ground surface at the well enclosure and battery rendering the enclosures unsecure. An empty 55-gallon drum and chemical pump should be removed if not used. The 8.625-inch casing is not exposed for monitoring as required. In addition, the following safety issues were noted. Our inspection was for Urban Preliminary Restoration of this well site. Access Road is crushed, packed limestone and brick too well head and tank battery area. Access road gate is locked and secure. The wellhead is clean and neat with no signs of problems or issues. Well head is secured within a board on board fence which is locked and secure. The tank battery is located within a board on board fence which is locked and secure, and is a shared tank battery with the Oyler D 1 unit P# 2-2003 and the Oyler 3 D unit P# 2-2126. Each well has its own separate tank unit system. System consists of two 140 BBL storage tanks and one 70 BBL brine tank. Storage tank lids are closed and secure. Tanks are properly vented with a tank vents and a charcoal filter unit. There is one vertical and one horizontal separator unit for the well as well as pick up lines which are secured with locks and bull plugs. The well is equipped with metering runs. The well is in compliance with 1501:9-9-05 and preliminary restoration is passed. |
