The well is located on the Temple property. During this inspection, I met with Mr. Shankman at the location. According to Mr. Shankman, there is an area near the existing wellhead where the ground surface has heaved up, affecting the asphalt-paved parking area and driveway. Upon inspection I noted the preence of an area to the immediate northeast of the wellhead, with approximate dimensions 75 ft. by 50 ft. characterized by longitudinal, rolling upward dispacement or heaving of the ground surface. The affected area coresponds with the former location of the drilllng fluid re-circulation pits. Mr. Shankman indicatd that this area had experienced similar heaving several years ago, requiring mitigation by the previous owner of the well, Seagull Development Corp. I had previously noted the heaving condition in an inspection conducted earlier this year and reported to the well tender for Seagull. At that time, ownership of this well was in transition from Seagull to Baay Oil. Based on these findings, I contacted Brooks Yarbrough with Baay Oil and advised about the condition and indicated that I was requesting a site meeting to discuss mitigation. |

At the time of this inspection, I observed the following: Ingress and egress from the well area is afforded by the shared asphalt-paved drive and parking areas for the Suburban Temple Kol Ami. A wellhead with mechanical plunger lift system is located in a chain-link enclosure within a grassy strip in the east parking area. The well enclosure was locked and secure. Fittings and connections at the wellhead were secure and free from leakage. The 8.625-inch casing annulus was gauged and showed zero pressure. The wellhead enclosure is surrounded with protective bollards and evergreen shrubs. The shrubs showed some signs of deer predation. Areas around the wellhead consist of turf grass which was free from signs of stress or impact. The tank battery is located 150 feet northeast of the wellhead on an elevated grassy area just off the asphalt-paved parking. The battery is comprised of one 100-barrel, low-boy steel production tank, one 50-barrel poly brine tank. At the time of this inspection, the access gate to the battery was locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. The load-outs are located remotely from the battery, at the edge of the parking area and were plugged and locked. Evergreen screening was present surrounding the battery and is good condition. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation. I did observe the following issue pertaining to restoration of the buried flow-lines and former drilling pit area. These areas were exhibiting a very significant amount of heaving of the soils and asphalt paving covering the flow-lines as they traverse the parking area and former pit, just northeast of the |

The site meeting was conducted pursuant to a complaint by Mr. Shankman (Complaint # 5374) that the former location of the drilling pits has exhibited severe heaving rendering a portion of the parking area unusable. Mr. Shankman reports that the heaving had occurred previously several years ago, resulting in a re-grade effort conducted by the former well owner/operator Seagull Investments. Mr. Shankman reports that the heaving was noted to be occurring again about 18 months ago. At the time of the time of this inspection, I observed an area northeast of the existing wellhead which had heaved up, displacing the asphalt pavement upward by as much as 24 inches. This heaving was observed to trend in a linear fashion SW to NE, coincident with the former pits. Mr. Yarbrough suggested a course of action which included removal of the heaved soils and re-grade level, with a temporary gravel pavement, pending a period of observation to assess recurrence of the heaving. Mr. Shankman indicated that the placement of gravel paving on a temporary basis would require approval by the Temple board of directors. Mr. Yarbrough indicated that he would begin the process to arrange for contractors to being the regrade work within the next 2 weeks, with the provision that ODNR DOGRM would review the work in progress and perform an engineering evaluation of the subsurface material and methods as deemed appropriate. |

Reportedly, the work crew reports that there may be only one truck available to haul removed soils, and bring new backfill. In the event that this schedule cannot be met, he had requested that the activty be postponed unit after the weekend. Due to the lack of truck availability, they elected to postpone work. I spoke with Brooks Yarborough of BAAY OIL and he agrred to advise when the schedule to complete the work was determined. |
