0394480050 | 2018-03-22 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity. The well and battery are located on an otherwise undeveloped parcel . At the time of my inspection, I found the following: wellhead with pump jack system and associated battery in separate board-on-board enclosures. The enclosure gates were locked and secure. The fittings and connections at the wellhead and battery were secure and free from problems. The interior of the battery, including secondary containment was free from free oil, staining, or other indications of spills, leaks, or problems. The battery is shared with the Callas #1, located nearby. The electrical panels and load line valves were locked and secure. I walked and inspected the areas surrounding the well and battery and found no indications of impacts or problems relating to the on-going production activity. The access road gate had been left open at the request of Cutter Oil due to the snow being plowed into the entrance and making access difficult for the water haulers. |

1225517458 | 2012-06-19 | UrbanProduction Wells | No Violations | Company is making repairs and upgrades to this tank battery in an effort to limit the number of venting off problems with this well. Plumbing of the well will be routed through a line from the compressor pop off valve directly into the separator. Any pressure on that line exceeding 500 PSI will feed directly into the storage tank. By making the connection in this manner, the problem of excess venting should be more easily controlled. |

1292697038 | 2015-05-06 | UrbanProduction Wells | Violations Noted | I inspected this well location at the request of the North Royalton Mayor. The well is located in a clearing within a primarily wooded area at the terminus of Mercury Drive. The entry to the well is gated with a cable-style gate which is locked. The access road is gravel-paved and accesses two wellheads (Callas 1 and 3D). At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The shared tank battery is comprised of a total of four 140-barrel steel production tanks, two 50-barrel poly brine tanks, and two mechanical separators. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. ID signage is needed at the entry gate. Evergreen screening is stressed and root balls are exposed in places. A discarded sump pump was observed on the north side of the battery. There is some oil leakage at the stuffing box. Signage is present at the battery but lacking complete information. Signage at the wellhead is falling off. The 8.625-inch casing is not exposed for monitoring as required. Gravel material from the lease road is being dragged onto the roadway by well maintenance traffic and requires management. In addition, the following safety issues were noted. Load lines are not locked. Production tanks are missing lightning arrestors. The dike drain has been left open. These safety issues must be remedied as soon as practicable or by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding this Compliance Notice. |
1407884791 | 2018-01-26 | UrbanProduction Wells | No Violations | I inspected this well as part of routine inspection. Fittings and connections at the well and battery (shared with the Callas 1) were secure and free from leakage or problems. Load lines were plugged and locked. Tank lids were closed. The secondary containment was intact and free from standing water, or signs of leakage of petroleum fluids. The individual enclosure gates were locked. Bonding and insurance were current. Production reports were current and up-to-date. The well was in production with active chart record. The shared-use access road was in good condition. I walked and inspected the areas surrounding the wellhead and battery and found the natural greenscape to be free from signs of impact or degradation. |
1495725199 | 2017-04-25 | UrbanProduction Wells | No Violations | I responded to this well location pursuant to a report from the ODNR emergency response operations center of an uncontrolled release of oil. On my arrival, I met with crews from Cutter Oil Co. (Cutter), Fred Romeo (ODNR Emergency Response), Wade Balser (OEPA), Thomas Habak (N. Royalton Asst. Fire Chief), Nick Cinquepalmi (Service Director), and Mark Schmitzer (City Engineer). Based on our discussions, the following had occurred: an estimated 20 barrels of crude oil escaped from a 4-inch cross-over fitting connection in the battery. The connection failed and became decoupled for unknown reasons. An estimated less than 20 gallons of oil escaped the secondary containment from a point on the north side of the containment dike and entered the adjacent storm water retention pond. An initial evaluation had indicated that the oil was restricted to the limits of the pond and was not observed to have migrated beyond the pond via the outfall structure located at the northeastern quadrant of the ;pond. Initial response by the N. Royalton FD and Cutter Oil included placement of 200 feet of oil sorbent booms and pad material in the pond to contain and begin recovering the free-phase oil. During my inspection, mitigation and recovery work continued and included the following: Wade Basler deployed a hard boom along with a sorbent boom at the outfall. Cutters crews continued collected free oil form the surface water. Cutter brought in a vac truck and began recovering free oil from the battery interior. Crews also excavated a shallow cut-off trench immediately down-gradient of the battery drain outfall, and constructed a temporary weir dam to prevent any additional oil seepage from entering the pond. A discussion with Cutter indicates that the last visit to the well location by a tender/pumper was on Friday 04/21/2017. So the initial occurrence of the leak was some time between 04/21/2017 and 04/25/2017. By the end of the day, most of the recoverable free phase oil and been removed |
1496547813 | 2012-11-30 | UrbanPreliminary Restoration | No Violations | My visit to the well today was to perform an Urban Preliminary Restoration Inspection. The access road is built with crushed limestone packed to a depth of six inches. The wellhead is enclosed in a board on board fence, secured with locks and chain. The well head area is neat, clean and free of debris and liquids. The tank battery is a shared location with the Callis #1 well, Permit #2-2085. The battery is enclosed in board on board fencing and is clean, neat and free of debris. There are two 140 BBL storage tanks and one 35 BBL brine tank. The tank caps are in place and secured. There is a vent system installed on the tanks and a charcoal filter is installed. There is one vertical and one horizontal separator in place and a metering run is installed. Pick up lines are located inside the gate of the tank battery and are secured with locks and bull plugs. Landscaping and restoration of the entire site is complete. No violations were found and restoration inspection passed. |
1675752846 | 2013-10-28 | UrbanProduction Wells | No Violations | The producing unit consists of a wellhead with pump jack and above-ground storage tank (AST) battery. The battery is comprised of 4 140-barrel steel production tanks, 2 50-barrel brine tanks and 2 separator units with Dominion metering run. The battery is shared with the #2085 unit. Both the wellhead and battery are enclosed in separate board-on-board fences. The gates and load-outs are locked. ID placard and secondary containment are good. Process safety gear (spark arrestor, low-pressure relief valve, carbon filter) is present. No problems noted. |
1729788549 | 2019-05-06 | Production Wells | No Violations | I conducted an inspection of the well and found no violation at well or tank battery.
The following maintenance issues need to be repaired: Gaps between the top of the dike and bottom of the board on broad fencing around the dike. I contacted Ed Cutright (contractor pumper) and he stated it will be put on the repair schedule. |
1806248018 | 2018-10-17 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection and evaluation of current condition following recent change in ownership. At the time of this inspection, I met with EOS Energy Representative Mike Corp and North Royalton Fire Chief Chegan. I noted that the well was in active production. The enclosure gates were locked and secure. The identification was current and accurate. The load-line valves were plugged and locked. Tank lids were closed and sealed. I walked the areas surrounding the well and battery and found no signs of problems or indications of I mpact. I noted the following maintenance items that required attention: the gate at the access road was not secure and required proper signage. He indicated that he would have the issues addressed as soon as possible. |
1823475499 | 2015-02-10 | UrbanProduction Wells | No Violations | I inspected this producing unit as part of routine inspection activity. The well and battery are located at the terminus of a short lease road which begins at the end of Foxwood Dr. There is a short dedicated access road which travel west from the end of Foxwood Drive, and curves slightly northward. The Callas 1 (#2-2085) is co-located with the Callas 3D (#2-2163) and share a common tank battery. The wellhead pump jack and battery are located in separate board-on-board enclosures. The enclosure gates are locked and signage is present. Observable fittings and connections are secure and free from signs of leakage. At the time of this inspection there is 12 to 15 inches of snow cover which limits my ability to inspect the condition of the lease road, interior of the battery containment, and other surficial features. |
1930056240 | 2015-06-25 | UrbanProduction Wells | No Violations | I inspected this well as a follow-up to a previous Compliance Notice. Load lines are now lock-protected. Tank lids are sealed and secure. Electrical boxes are locked and secure. Process safety gear, including spark arrestor is present. Secondary containment at the well and battery are intact. The dike drain is secure. The trash and other debris previously noted has been removed. The mounded earth noted previously has been removed. The hole at the end of the drainage culvert of the east side of the access road has been back-filled. Update of signage is in progress. There have been reported complaints that storm drainage from the gravel-paved entry apron at the end of Foxwood Drive is causing gravel to wash into the local storm sewer. Ive advised Cutter Oil to establish a management solution for the drainage. Since the time of the last inspection, the loose gravel has been removed. This will likely require on-going management by the well owner. |
2132253647 | 2014-07-10 | UrbanProduction Wells | No Violations | I inspected this producing unit as part of routine inspection activities. The wellhead which is currently fitted with a pump jack system is in operation. Production reports are current. System operation at the above-ground storage tank (AST) battery appears normal. Safety apparatus are present and I observed no signs of leaks, spills, or problems. |
