At the time of this inspection, I observed the following: Ingress and egress from the well area is afforded by the shared asphalt-paved drive and parking areas for the City of Beachwood Service Garage. A wellhead with mechanical plunger lift system is located in a chain-link enclosure at the eastern edge of the parking area. The well enclosure was locked and secure. Fittings and connections at the wellhead were secure and free from leakage. The 8.625-inch casing annulus was exposed with pressure-relief fitting. Areas around the wellhead consisted of turf grass which was free from signs of stress or impact. The tank battery is located 50 feet north of the wellhead on an elevated grassy area just off the asphalt-paved parking. The battery is comprised of one 100-barrel, steel production tank, one 50-barrel poly brine tank. At the time of this inspection, the access gate to the battery was locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. The load-outs are plugged and secure. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the produciton operation. Surface drainage for the area is to a storm sewer catchment located adjacent to the battery. I did observe the following maintenance issue: the load-out valves must be locked. |

Hole was 7 7/8 inch TD at 3766 feet. Top of the Lockport was 3050 feet. Operator ran 3739 feet of new R-3 11.6 pipe. Fill up needed 816 feet, 816 X .2278 / 1.18= 157 sacks of cement for fill up. Operator ran 230 sacks of Class A 2% CaCl. Operator broke circulation, mixed cement, ran cement, displaced and locked plug down with 1500psi. No loss of circulation noted during cementing. Cement water was from a municipal water supply, pH 7. Operator placed 7 centralizers on the long string 1 st centralizer was placed on the pup joint then every other joint after that. Operator requested that they be allowed to solidify pits then dig a 2nd pit in the bank were the rig is located and encapsulate drill cutting there. Company was given two options: preferred option is to solidify pits and remove from site. Operator was given contact information to Waste Managements Geneva land fill, the land fill will take PCS waste. 2nd option solidify pits and construct 2nd pit in bank. Operator was informed that current pits do not meet (1501:9-3-08 standards.) Operator excavated pits and built a 4 foot berm to increase the capacity of the pits. Water level was above natural ground level. |

Production records are current. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located along the perimeter parking area for the City of Beachwood Maintenance Garage. The wellhead is enclosed in a chain-link fence which is locked. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located north of the wellhead, along the same perimeter area. The battery is comprised of 1 steel 100-barrel production tank, a 25-barrel poly brine tank, and a horizontal separator. The battery is located in a separate chain-link enclosure which is in good condition. The load-out is bull-plugged and locked. The secondary containment at the battery appears is well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery which is in need of maintenance. Process safety equipment is present. The area of the well is generally level with a slight topographic drop to the west. Surface drainage is primarily controlled by engineered storm sewer infrastructure and drain inlets, of which there is one within several feet of the battery. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by general entry into the maintenance garage area, and there is no dedicated access route or roadway. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are comprised of advanced development for commercial use. Numerous multi-tenant commercial and professional offcie buildings are located along Mercantile Drive. I notified Seagull about the need for maintenance on the ID signage. |
