0014286474 2017-10-19 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The well and battery are located on a parcel used by the Cuyahoga Valley Career Center. At the time of this inspection, I observed the following: a wellhead with linear rod pump system which was in operation. Fittings and connections at the well were secure and free from signs of leakage or problems. The chain-link enclosure was locked and secure. The associated tank battery is located in a separate chain-link enclosure which was locked and secure. The battery is shared with the #1 well located on the same property. The fittings and connections were secure. The secondary containment was intact and dry. Signage was present. I walked the areas surrounding the well and battery and found no signs of degradation or impact to the natural greenscape.
0303485785 2014-08-04 Urban Drill / Deepen / Reopen No Violations I inspected this well location during routine post-drilling completion activities. Crews from Producers Service Corp. are staged on site for well stimulation activities, which include application of hydraulic fracturing. Existing equipment includes truck-mounted pump and container apparatus, sand trucks, and staged 300-barrel frac tanks. I observed fracking procedures to proceed according to the prescribed plan. Back side annular pressures were monitored and remained static. Following completion, flow back will be executed utilizing the staged frac tanks.

The gravel-paved access road to the wellhead and battery was dry, and in good condition. The wellhead pump-jack system was in operation. The fitings and connections were secure. Secondary continament at the battery was intact and dry. I wlaked the areas around the well and battery and found the natural greenscape to be free from signs of impact. Production reports were current and there were no other problems or issues noted.

Preliminary restoration work is still in process. Some additional temporary fencing remains. The tank battery is shared with the existing CVCC#1 (#22150). The battery is comprised of 2 100-barrel steel production tanks, 1 100-barrel brine tank, two separators. Gates and load-outs are locked. Secondary containment and unit ID is good. Will approve preliminary restoration when complete.

1404327609 2014-07-02 Urban Drill / Deepen / Reopen No Violations I inspected the location of this producing unit as preparations to begin drilling are completed. Currently, construction and development of the access road is in progress. The accesss road will consist of a side-branch from the existing access for the CVCC #1 (#2-2150) unit. The access point is located in the eastern portion of the parking area for the Cuayhoga Valley Career Center. The head of the access road is currently gated with signage. The existing acesss for #2-2150 travels north from the parking area and then east to the wellhead and battery. The newly-established access road for #2-2207 will extend northeast from the existing acess. At the time of my inspection, the road has been cleared, graded and crushed limestone gravel is being applied. Work on the well pad area has not commenced. There is a gentle topographic drop to the northeast, in the direction of travel, calculated to be 2% on average. Surface drainage from the road and well pad area is to the northwest. I will return to re-inspect as work continues on the road and pad.
1405347772 2014-07-14 Urban Drill / Deepen / Reopen No Violations I visited and inspected the location of this unit during on-going drilling activities. At the time of this inspection, 11.75 conductor casing has been cemented to surface and the target depth for surface casing has been reached. The 8.625 casing has been set and crews are prepping to cement to surface. Circulation was established with no appreciable losses or noted production of gas. The cementing of surface casing was completed with good returns. I observed fluid levels in the pits to be good with ample freeboard. Hole conditions will remain static for a minimum of 8 hours until installation and test of the blow-out preventor.
