0521547204 2018-03-19 Orphan No Violations I met with the landowner at the location of this suspected well. Upon my inspection, I noted the following: a round, decorative masonry water well-style enclosure with a closed lid on the top with a 3/4-inch steel vent riser (presumed) extending upward for several feet. The interior of the structure was not accessible. There were intermittent gas odors apparent, but no sustained, meter-detectable amounts. The structure had an apparent 3/4-inch supply fitting on the side, at grade. Nearby was an apparent former gas-light. The landowners report that the house on this parcel was connected to Dominion Energy for gas, and had no knowledge of any existing or former connection to alternative sources of gas. I advised that the structure would need to be removed to further assess whether a gas well was there, and what condition it was in.
1175146373 2018-03-26 Field Inspected, Well Not Found No Violations Conducted inspection due to a complaint of gas odor located at address 14714 Bayes Ave. This posted location on the emergency well locator. I visually checked the backyard of address 2173 Warren road Parcel number 314-20-037 for any field evidence of well equipment or dead areas which would indicate a plugged well leaking to ground level.

No field evidence of any well at the Lat/Long posted in the RBDMS data system.

1523293643 2018-04-02 Orphan No Violations I inspected this well location as a follow-up to previous activity which included dismantle of a decorative well structure and installation of a temporary, passive vent stack. At the time of the activity, which was performed on 3/26/18, an approximate 10-inch diameter uncased borehole was identified, at a depth of 5 feet below unconsolidated soils. At the time of this inspection, a 2-inch diameter PVC vent stack riser was present at the location of the borehole, which was otherwise re-buried. Gas odor was apparent near the vent stack.
