The well is located in a small, wood-sided maintenance shed next to a multi-story, apartment building. At the time of this inspection, I observed the following: a wellhead with dual-flow manifold system attachment to Dominion EOG supply line. There is no apparent metering system for the well-side flow line. Fittings and connections at the well were secure and free from signs of leakage or problems. |

The well is located adjacent (4 feet) from the south exterior wall of the apartment structure, and housed within a wooded lean-to style attached structure. Well elements consisted of a 3-inch casing riser with reduction swages to 2-inch and 1-inch supply lines and other appurtenances. I met with the facility manager for the building (Mr. David Mantifel). He reported that Doug Hottle had been performing routine maintenance on the well, which supplied gas to the building via a dual manifold connection shared with Dominion East Ohio Gas. At the time of my inspection, the well was in active use, with 75 PSI indicated at the wellhead. I did not note any signs of leakage or problems. The well connections and fittings were secure, with no gas odors or meter-detectable gas readings in or around the well enclosure. The surrounding areas were comprised of asphalt-paved parking and driveway with no signs of impact or degradation. |
