0239255444 2016-12-14 Plug / Plug Back No Violations I was on location to witness on-going plugging and abandonment being conducted under contract with Idle & Orphan Well Program. At the time of my visit, crews from Moore Well Service were rigged up at the well location with the following equipment: double-axle service rig, with 50 mast erected, a trailer-mounted mud pump, a pipe trailer, and a 20K -gallon frac tank with kill fluid. Drilling had commenced utiizing a tri-cone roller bit. An obstruction had been encountered around 80 feet below grade, consisting of loose shale and cuttings suspected to be remnants from previous drilling. Fluid returns were good with no reported losses and little gas production, but with trace amounts of oil and oily cuttings. There was no appreciable gas odor or meter-detectable gas being produced from the well. Drilling of the obstruction was steady at 120 feet and progressing.

1333300681 2015-08-12 Orphan No Violations This follow-up inspection was conducted on 08-12-15 to confirm the well status prior to tomorrows planned excavation of the well area. I met with Ralph Isenhart, the property manager, on site. He proceeded to show me the well vent, which was broken at ground level and laying on the ground. While still on site, the person hired to do the backhoe work arrived and I proceeded to show him the area to be excavated. I advised him that the landowner was still waiting on OUPS to mark the utilities in the work area.

I proceeded to check area to be excavated with my Magnetic Locator and fobtained a reading two feet to the east of the vent pipe. No other reading were obtained in the work area.

1457528469 2016-03-08 Orphan No Violations Met onsite with landowner, Donna Anderson. This property was a former mobile home park and is in the final phase of having the property cleared of all the trailers and out buildings. Allegedly, there is/was a 2nd well located on this plot of land. Numerous spots explored and a metal detector was used, with no firm results. I told Ms. Anderson that when the clean up is completed, to call and I would return with metal detector to try to locate the alleged second well.

1789608098 2018-05-08 Orphan No Violations I inspected this former well location pursuant to a request from the Fire Chief of Westlake regarding an inquiry from the property development contracting company. The company had expressed a desire to re-route the passive vent stack risers to accommodate future construction concerns and to protect the risers from damage. I advised that the existing oil and gas rules and regulations (Ohio Revised Code 1509 and Ohio Administrative Code 1501:09) make no provision or specification for post-plugging vault configuration. I directed the contractor to seek guidance from the local fire department and/or local building zoning for any directives.
1901469747 2016-12-13 Orphan No Violations I was on the well location to observe plugging and abandonment activity which was being commenced under the Idle & Orphan Well Program (I &O). Also on location at the time of my visit were I & O inspectors Perry and Keppler. I met with Inspectors Keppler and Perry and we discussed the plan of activity for the day. At the time of this inspection, the plugging subcontractor (Moore Well Service) had staged their plugging rig at the wellhead, along with a open-top steel recirculation pit. The area around the well was recently leveled and graded, and comprised of bare soil. A threaded collar had been welded to the existing nominal 7-inch casing and a reducer swage with valve attached, and left open. There was no meter-detectable gas near the open wellhead valve. A tempoary cellar strcuture had been created using a section of pre-fabricated plastic culvert pipe. The interior of the cellar was dry. Further activities planned for the day included attachment of a control head at the well and preparation for loading the hole and drill-out procedure.
1932295594 2015-08-13 Orphan No Violations This follow-up inspection was conducted on 08-13-15. I met with the property manager, Ralph Isenhart, who advised me that OUPS had been onsite and marked the utilities and also that Dominion was on site marking their lines, which are all to the north of the excavation site.

Steve Tomkins (DOGRM Cuyahoga County Inspector), Gene Chini (Orphan Well Program Manager) and Donna Anderson (Landowner) were also onsite. The contractor excavated immediately to the south of the well vent and extended the excavation approximately 5 on either side of the vent, 10 to the south, 6 to the north and to a depth of 8. The metal catch basin that was present during the initial excavation over a decade ago and believed to be the actual location of the well, was not found. Therefore the well casing present in the excavation, which was originally thought to be a water well associated with the drilling of the oil and gas well, is now believed to be the oil and gas well. The portion of a 55 gallon drum, installed over the 7diameter casing as a vault during the initial excavation, was removed. I used a weighted line an found the well casing to be open to approximately 90. There was a faint natural gas odor present in the air. This excavation will be left open and the area barricaded for safety.

2064492106 2015-06-10 Orphan No Violations I inspected the well as part of site update for orphan well program status. The well is located in a small open area of a trailer park. The well is partially concealed by a circular garden installation. Visible elements consist of a steel 2-inch riser which extends several inches above ground surface. A 2-inch PVC vent riser is affixed to the steel riser. The vent stack extends to an approximate height of 12 feet above ground surface. I detected some gas odor near the vent stack. No other well elements are visible above grade.
2128549448 2016-03-08 Orphan No Violations Met with landowner to locate 2nd well on property. Utilized old maps that landowner had in her possession, as well as a pin locator. After having explored the area, based upon the landowners records, it was determined that a second well apparently does not exist on this property.
