1341908308 2015-04-15 Orphan No Violations This well was previously reported by the Cuyahoga Valley National Park staff. The well is on the hillslope above the Cuyahoga Scenic Railway and the Cuyahoga River. The fluid and vapor recovery system which was put into place by Moore Resources is intact and secure. I observed oil accumulation in the retention system at the wellhead but the level remains below the drain point whereby recovered oil would travel to the poly tank. All connections and fittings of the system are secure at the time of this inspection.
1408632284 2014-08-21 Orphan No Violations I inspected this well location as part of a routine status check. This well was discovered previously as part of a joint effort by ODNR and Cuyahoga Valley National Park staff pursuant to odor complaints and oily leakage. Since the time of discovery, the CVNP has contracted Moore Well Service to perform some limited mitigation and application of temporary engineering controls at the well site. Mitigation has included excavation of a small amount of oil-impacted soil from around the wellhead. This material has been temporarily stockpiled and covered near the wellhead. Temporary engineering control has included the construction of a temporary vault and vent structure. The vault is comprised of a 30-inch diameter black corrugated pipe with fabricated metal lid and 2-inch passive vent. A 1,000-gallon poly tank has been stage down gradient with a 2-inch flow line to capture accumulated fluids. At the time of my inspection, I measured approximately 20 inches of fluid within the vault. The fluid is comprised of mostly water with approximately 2 to 3 inches of floating oil. The fluid level remains below the outlet and therefore no fluid has accumulated in the poly tank. The structure appears stable and functioning according to design.

2108258200 2013-04-03 Orphan No Violations I met with D. Strickland (CVNP) and Doug Gonzalez (Gonzoil) to evaluate possible short-term measures to stabilize the wellhead from leaking oil and/or gas during the interim period until the well is plugged and abandoned. The well does not appear to be leaking any gas, but is leaking a small amount of oil, which is localized to the wellhead area. Doug recommends a limited excavation to expose the wellhead structure before any attempts are made to investigate to the leak. We agree that sufficient preparations should be made to contain any fluids which might be produced from the well during excavation. Doug has agreed to begin developing a proposal to do this work, and provide it to CVNP.
