0071149768 2012-10-24 Orphan No Violations A follow-up inspection conducted with Jeff Fry revealed no change to the well status since the last inspection. The well consisted of 8 drive pipe, 6 surface casing and 4.5 casing. The 4.5 casing was approximately 3 above ground surface and contained a threaded cap on top of the wellhead with a fitting attached to a gate valve on the side. There was no connection between the gate valve and a separator located beside this well.
0153453298 2012-09-17 Orphan No Violations A follow-up inspection conducted with Mr. Rich Pennington revealed that the well consisted of approx 8 drive pipe with 6 (outside diameter) casing containing a flange and 4.5 casing equipped with a tee and gate valve installed on the top. In addition, a drip was also observed next to the well. There were no dishcarge of fluids or gas at time of inspection. The well is situated on a property designated as agricultural field (bean field) with no observed production lines at the surface or overhead obstructions.

When first inspection on 10/13/05, digital camera setting had mistakenly been changes to the movie setting and grainy movie taken of the well. Photos taken while on site. No one on site at the farmhouse. Field recently plowed around the well.

1129572354 2005-10-13 Orphan No Violations -On site after complaint # 1247 inspection for I/O by landowner Richard Burns across Reed Road. Met w/ Mark Dahlinghaus, one of the trustees for the Donald J. Dahlinghaus, Trust property at 10459 Reed Road. Three I/O wells on site. Mark indicated the 3 wells on his property was tied in with the Bruns well on west side of Reed Road. Mark indicated that he believed the well was drilled by former neighbor Linus Poeppleman (now deceased) about 50 years ago. Well record for well # 20009 shows 3 wells drilled by Poeppelman Gas Co. in 1954. All wells on the property are gas wells. Well # 3, the N-most well, has 8, 6, & 4 casing which sticks about 3 above the ground. A small collector tank on the well. The well does not appear to be plumbed up. Well has 1.5 production line running 30-40 above ground before being buried. All 3 wells are connected according to Mark. Well located in crop field. No obstructions. 3 videos/photos taken. Mr. Dahlinghaus left with Right-of-Entry forms to complete and submit. This well is well # 34-037-20009-00-00.

1628439143 2017-07-31 Plug / Plug Back No Violations Observed the removal of approximately 310 feet of 4.5 inch casing.
1639797657 2012-12-06 Orphan No Violations A follow-up inspection conducted with Orphan Well Program Supervisor, Mr. Gene Chini revealed no change to the well status since the last inspection. There was no discharge of fluid or natural gas or natural gas detected with a meter. As previously mentioned, the well consisted of 8 drive pipe, 6 and 4.5 casing equipped with a wellhead. At the conclusion of this inspection, an odor resembling natural gas was smelled while walking to the Southwest at the following coordinates: N 40.33020 W 084.48168. This location was approx 1200 away from Permit No. 9 and 700 away from Permit No. 60035. Note: The wind was blowing to the Northwest. A gas detector was utilized around the subject area, but showed no detection of gas. A metal detector was also utilized and revealed no relevant sign of steel located below the surface.

1799407655 2012-06-28 Orphan No Violations -On site per request by Terracina Maxwell in Columbus office to check on status of this well & 2 other wells on proerty after call from Mark Dahlinghaus re: status of the planned plugging under I/O program. This well & other 2 on property (and 1 on adjoining Bruns property across Reed Rd.) added to I/O program in late 2005 according to Dave Hodges in past. Initial complaint was # 1247 received 10/4/05. Mark indicated in his call that this well & the other 2 wells were getting worse. Well is a gas well. Well has 8, 6, and 4 casing wiich sticks about 3 ft above the ground. A small collector tank on the well. The well does not appear to be plumbed up. Well has 1.5 production line running 30-40 above ground before being burined. At time of 10/13/05 inspection, Mark Dahlinghaus indicated that it ran to the SE to connect with the central well and the well was along tied to the southern well. Well in soybean field. No significant change noted at the surface since 3/15/06 inspection. Photos taken. On site to check well & update priority matrix. Talked to Terracina by phone after inspection.
