1355319952 2012-12-06 Orphan No Violations A follow-up inspection conducted with Orphan Well Program Supervisor, Mr. Gene Chini revealed no change to the well status since the last inspection. There was no discharge of fluid or natural gas detected with a meter. The well consisted of 8 drive pipe, and 4.5 outside diameter casing (OD). As previously mentioned, an additional casing string may exist in the wellbore, as steel was observed approximately 1 down from the surface and wedged between the 8 and 4.5 casing annulus. The steel resembled a 2 collar (approx) and appeared to contain additional steel inside. RBDMS records reference 4 casing cemented but no mention of sacks utilized. Also, a Larkin packer was also shown to have been placed in the well. During the inspection of all wells located on this property, the landowner, Mr. Donald Schmitz confirmed the installation of field tile. Mr. Schmitz stated that the tiles were placed 20 apart and routed to the north and south. Steel tanks will be utilized in place of earthen pits. Mr. Schmitz requested for casing to be cut and buried 4 below ground level. A lease road does not exist for wells located on this property. However, the best access to the wells would be a gravel entrance to the field located off N. Star Fort Loramie Rd and not Lightsville Northern Rd.
1530630317 2018-07-03 Final Restoration No Violations Final restoration is not approved. Plugging contractors equipment has been removed from site and wells have been cut off and backfilled. Plugging contractor will need to grade driveway used for access before final restoration will be approved.
1569847151 2012-09-17 Orphan No Violations A follow-up inspection revealed that the well consisted of 8 and 4.5 OD (outside diameter) casings. The 4.5 casing contained a welded cap on top equipped with a fitting and a gauge no longer working. The annular space between 4.5 and 8 contained hard material possibly concrete approx 1 below the surface. The well card indicates that well should contain 8 and 6.25 casing. The well card had indicated 6.25 casing, but was not observed at this time. There were no fluid or gas leakage at time of inspection. A water well associated with the drilling of this o/g well was observed 18 away. 9 (OD) or 8.5 (ID) casing approx 2 above surface equipped with a removable lid. The fluid was down approx 250. The well is situated on a property designated as agricultural field (bean field) with no overhead obstructions. An access/lease road will need to be constructed and may require a culvert at the entrance. Note: Well location(s) were previously scaled and have now been corrected with GPS readings.
1608052774 2012-10-24 Orphan No Violations A follow-up inspection conducted was conducted with Jeff Fry to further evaluate Permit No. 15 and verify the total depth of a potential water well that would have been associated with the drilling of this well. During the inspection, the wellbore containing only 8 casing and located 18 away from Permit No. 15 was confirmed as a water well. The total depth of this well was measured total depth at 252 with a heavy weight and fishing line. Note: casing stick-up was 2. Permit No. 15 consisted of 8 drive pipe and 4 outside diameter (OD) surface casing. An additional casing string may exist in the wellbore, as steel was observed approximately 1 down from the surface and wedged between the 8 and 4 casing annulus. The steel resembled a 2 collar (approx) with 0.5 (approx) steel inside. The area surrounding this steel was probed and revealed no success in determining additional information. The 4 casing was also verified to contain no movement and was solid in place. This casing may have been cemented to the surface. The RBDMS well card was reviewed prior to the inspection and showed that the 4 was cemented with no mention of sack number. Furthermore, there was also mention that well contained a Larkin packer.
1733797716 2018-05-31 Plug / Plug Back No Violations Plugging contractor ran approximately 670 feet of 1.5 inch tubing into well. Observed the installation of 100 sacks of Class A Cement to this well and cement was observed at the surface.
2144774055 2014-02-03 Orphan No Violations This inspection was done to review the location to complete the plugging Scope of Work for the Orphan Well Program. There was no change in the status of the well from previous inspections. All GPS readings were verified and updated pictures were taken.
