1078841825 2004-03-04 Production Wells No Violations -Site inspection with Jeff Fry of the Joshua & Sherrie Norris (100 Sluterbeck St., Arcanum, OH 45304, ph. 937-692-5476) domestic gas well per request of Jim Francis of Vectren Gas Co (e-mail on 3/2/04). The Norris smelled gas & contacted the gas company. Norris purchased the house in fall 2003 & did not know there was a gas well leading to the house. Vectren found three leaks and tightened valves. Third valve is leaking small amount of gas w/ H2S smell. Sherri Norris indicated that there is a furnace in the house that is connected to the well but had not used gas from the well.

-Mrs. Norris was left with an idle/orphan well program pamphlet.

1131555998 2005-11-07 Plug / Plug Back No Violations -On site for planned plugging of the Joshau Norris I/O well under the 2005 landowner grant program. Met with Bob and Adam Huwer of Huwer Drilling Co. who has the plugging contract. On site at two different times during the day on 11/7/05. When at the site, Bob had removed a regulator and was moving the cinderblock stacked around the conveyor. Well was connected to an furnance in the basement of the Norris house. Bob indicated the hole is 1050 feet deep with very little water in it. Well is leaking gas at the time and gas vented via hose away from the well. Surface casing is 7 inches with a 2 production line. When well was drilled, rope was used between the surface and production casing at the wellhead. Bob did not have the proper equipment to get the rotted rope out. Also Bob did not know how far down the production casing went but had planned to cut off. Bobs brother had a camera which he planned to borrow and run down the hole on 11/8/05. Production casing was to be retreaded and cap placed overnight. Talked to Megan and Sherrie Norris while on site.
1131557193 2005-11-08 Plug / Plug Back No Violations -On site for additional review of the Joshau Norris 2005 landowner grant I/O well plugging. Since yesterdays inspection, Bob has run a camera down the well. 2 production line runs to 230 of the 1050 deep well. Additional plumbing on the well removed that ran into the house. 5020 lbs of Class A cement (50 sacks) from St. Henry Tile pumped down the 2 production line in order to close off the gas leaking from the well. Bob Huwer will allow the cement to set up. The top of the 2 production line was retreaded and cap to be placed on it. Bob is schedule for drilling and will not be available later this week and will try to finish plugging the well next week. Rig may have to be brought in to pull the production casing. Bob said it appears that a packer is in the well. Bob Huwer to notify the Norris family that additional plugging activity will occur probably next week. Photos taken.
1193841327 2007-10-25 Orphan No Violations -On site for final plugging of well. Unregistered domestic well was leaking gas on 3/2/04. Well inspected on 3/4/07. Landowner applied for plugging under landowner grant program & permit issued 1/14/05. Huwer Well Drilling contracted to plug well. 50 sacks of Class A cement pumped down the well through the 2 production line on 11/8/05. Gas leak shut off. Some rope was used as a packer & Bob was going to get equipment to pull the 2 out in the next few weeks. Huwer finally called & scheduled the remainder of the plugging for 10/25/07. Additional 120 sacks of Class A cement pumped in the well. Cement circulated. Only retrieved 21 feet of the 2 production casing. Surface casing is 6 diamater & was cut it off at 2.5 feet. St. Henry Tile Co. was the cement contractor for both the 11/05 and 10/07 plugging. Plate with ODNR 6-26 was present. Bob to cap, fill around, & seed area over the well in the next few days. Notified Bob to photo the cap placement and filled in area and send copies of the photos. Also will go back up to view the well location in the next few weeks. Photos taken
1198846179 2007-12-27 Final Restoration No Violations -On site for final restoration inspection. Since last inspection on 10/25/07, Bob Huwer has cut off the well below the surface, capped it, filled in around the well location, and seeded the approx. 2-3 foot diameter area around the former well location. Well was an unregistered domestic well that was leaking gas on 3/2/04. Well first inspected 3/4/04. Landowner applied for plugging under landowner grant program-permit issued 11/14/05. Huwer Well Drilling contracted to plug well. 50 sacks of Class A cement pumped down well thru 2 prod. Line on 11/8/05 & gas leak shut off. Some rope used as a packer in well. Huwer did not get back to finish plugging well until 10/25/07 when addl 120 sacks of Class A cement was pumped & well was filled. Only 21 of 2 prod casing recovered. Surface casing & 6 casing cut off at 2.5. Cap w/ ODNR 6-26 was placed on well. Well debris has been removed. Grass is growing. Photos taken during 12/27/07 insp & final restoration rpt to be prepared.
