0923676209 2016-05-10 Production Wells No Violations Compliance notice follow-up. The well is encased in a 55 gallon drum that is filled with concrete. 2 pipe is sticking out of the concrete. The pipe has a 2 tee with a valve. The valve is shut off. There is also 1.5 pipe that is at ground level with a valve that is turned on. I observed a slight odor of gas. There is a pressure gauge that read 40 PSI. No one was present at the business were the well is located.
1081966062 2004-04-13 Production Wells No Violations -Site inspection w/ Jeff Fry per request of Jim Francis of Vectren Gas Co. Met w/ Gary Bechtol of Bechtol Plumbing (937-692-5817). Gary said he used to own the property but sold it to Garbic & Breckenridge (partnership). Well present on site providing gas to heat one of the buildings at the plumbing company. Guage indicates 17 lbs of pressure on the line. It has reached 40 lbs in the summer according to Gary. Well about 300-400 feet deep according to Gary. Slight leak notices. Casing embedded in 55 gallon drum. 2 line coming off well. Business card left w/ Gary & asked him to have the landowners contact office. Well will be required to be registered. Financial assurance & proof of liability insurance from the owner will be required. Digital photos taken. Temp. # assigned. Well is on Arcanum USGS topo map.
1280236584 2010-07-22 Production Wells No Violations -Complaint # 2779 received 7/21/10 from Kurt Troutline, Arcanum, OH fire chief re: gas leak at well located at 211 S. Sycamore St., Arcanum, OH 45304. Well inspected on 7/22/10 w/ Mike Mitchell and Kurt Troutline. Mikes gas meter readings were 20.6 for oxygen, 7 LEL, and H2S reading of 0. 22 lbs of pressure on line. Well encased in 55 gallon barrel filled with cement. Small leak in 2 line leading from well. Well used to heat machine shop on site. Talked to Rick (machine shop worker) who indicated he rented the site and gave me the owners phone # (937-692-5278). Kurt Troutline indicated the property was owned by Big Ridge LLC, P.O. Box 100, Arcanum, OH 45304. Landowner office called while on site & talked to secretary/receptionist Julie. Phil Garbig not available to take the call. Told Julie we were responding to complaint of a leak and that needed handled and the well needed registered. Photos taken of the well. Well was inspected on 4/13/04 with Jeff Fry and landowner called at that time but never called back. Landowners office looked up on 7/26/10 and info to be dropped off on 7/27/10.
1280239496 2010-07-22 Production Wells Violations Noted
1352465720 2012-10-24 Production Wells No Violations A follow-up to Rich Penningtons 2010 inspection was conducted with Mr. Jeff Fry. During the inspection, it was noted that the well ownership issue was not resolved, and RBDMS well status still showed Historic Owner. An interview was conducted with Mr. Rick Emerick, an owner of the machine shop situated on property owned by Mr. Phil Garbig, 937-692-5278 (work phone). Mr. Emerick leases the property from Mr. Garbig and does utilize the gas from the well. The well was enclosed in a 44 x 33 x 3 wooden box that did not allow for visual observation of wellbore. Gas was smelled and detected with a meter. The readings fluctuated therefore, the highest was recorded and was as follows: 02= 20.7, CH4-IR= 0.35, LEL= 6, CO= 2, H2S= 0. A slight sour gas was evident, but H2S was never detected on the gas meter. The property owner will be contacted by telephone to discuss well status and ownership.
