0289942480 2012-06-28 Orphan No Violations -On site for status check per request of Terracina Maxwell in Columbus office after inquiry from Mark Dahlinghaus re: wells on his property E. of Reed Road. First met Richard Bruns on 10/13/05 after his complaint # 1247 received 10/4/05. Bruns well was placed on I/O program in October 2005. Not plugged yet. Terracina wanted a status check and review well condition and update priority matrix for Bruns & Dahlinghaus wells. Well is located on east side of Deweese Ditch, about 300 south of E-W field road in conservation easement. Mulberry tree continuing to grow up around the well. Well casing bent by the tree. 2 prod. Line above ground across ditch. At 10/13/05 mtg, landowner said buried line on w. side of ditch. 7 casing on well. Grass to high to see if meter is still lying on ground as noted on 10/13/05 insp. Photos taken. Priority matrix to be updated. No oil seen on ground.

1130337860 2005-10-13 Orphan No Violations -On site per complaint # 1247 received 10/4/05 regarding I/O gas well. Joint inspection with Jeff Fry. Met w/ landowner Richard Bruns. Landowner bought house in 1994 from his uncle Ray Bruns. Well located along grass conservation strip on the east side of Deweese Ditch, about 300 feet south of E-W field road. Mulberry tree growing up around the well. Well casing bent by the tree. 2 production line above the ground across the drainage ditch. Landowner made a reference to buried line west of the ditch. Well has a 7 casing. Old meter lying on the ground. The well is tied to 3 other wells on the Dahlinghaus property on east side of Reed Road. Mark Dahlinghaus thought the 4 wells were drilled by Linus Poeppleman (deceased) about 50 years ago. Courthouse search saw no listing of any leases relative to the well. One of the Dahlinghaus wells (# 3) is listed on the state records as 20009 and listed as drilled by Poeppelman Gas Co. Good access to site. No overhead obstructions.

1220081178 2017-09-07 Plug / Plug Back No Violations On location with Jet Wireline Services to log well. Total depth of well was determined to be 1148 feet deep.
1350472079 2012-09-17 Orphan No Violations A follow-up inspection revealed that the well consisted of 8 drive pipe, 7 casing, and approx 2 tubing. The 7 casing was equipped with a wellhead. There were no leakage of fluids or gas at time of inspect. A mature tree (type- mulberry) has grown alongside casing and will require removal to operate. If following the trend of wells in this area, this well was most likely drilled to the Trenton formation (1200 -1225.). A section of approx 25 of 2 steel line was suspended above ground to cross intermittent stream and will require removal. Mr. Bruns stated there were no additional line(s) above ground, but underground line(s) did exist between this well and Dahlinghaus locations. Mr. Bruns would like only the visible steel lines to be removed. Additional oilfield debris consisted of an old gas meter and section of approx 1 line sticking up above ground. In regards to accessing the well location, a gravel driveway can be accessed at Mr. Bruns residence. From edge of this driveway, travel approx 450 in field to the well location. Along this path, the rig would need to avoid an area containing a ditch, which had recently been unearthed for fill material on property.

1994906244 2017-09-11 Plug / Plug Back No Violations Witnessed the installation of nine sack grout for top plug on well. Grout was observed at surface.
2032007867 2017-08-17 Plug / Plug Back No Violations Orphan well contractor began site work on this well. Two large trees were removed around well area with little damage caused to well casing or area around well. Erosion controls were implemented around well since well site is close to a small stream.
2048827025 2017-09-07 Plug / Plug Back No Violations Installed bottom plug on this well by pumping Class A cement through one inch tubing. Approximately 70 sacks of Class A cement was used. Orphan well contractor was able to remove all the tubing from well.
