0120721967 | 2012-12-06 | Orphan | No Violations | A follow-up inspection conducted with Orphan Well Program Supervisor, Mr. Gene Chini revealed no discharge of fluid or natural gas detected with a meter. As previously noted, the well was equipped with a molded concrete wellhead, which measured approximately 8 in diameter. Above this mold, an 8 bolted flange contained 2 gas fittings, and a gate valve before the connection into a nearby separator and regulator. The overall form of the concrete to area just below the flange resembled the shape of a swage. The 82 x 44 concrete walls will be removed from the location after plugging and abandonment. At the conclusion of the inspection on Permit No. 9, an odor resembling natural gas was smelled while walking to the Southwest at the following coordinates: N 40.33020 W 084.48168. This location was approx 1200 away from Permit No. 9 and 700 away from Permit No. 60035. Note: The wind was blowing to the Northwest. A gas detector was utilized around the subject area, but showed no detection of gas. A metal detector was also utilized and revealed no relevant sign of steel located below the surface. |
0655559423 | 2017-08-16 | Plug / Plug Back | No Violations | Orphan program contractor attempted to clean well bore with 3.5 inch liquid bailer. Was able to get to a depth of 1196 feet deep. Will determine at a later date if drilling will be necessary to deepen well. |
0989031535 | 2012-06-28 | Orphan | No Violations | -On site per request by Terracina Maxwell in Columbus office to check on status of this well & other 2 wells on property after call from Mark Dahlinghaus re: status of the planned plugging under I/O program. This well & other 2 on property (and 1 on adjoining Bruns property across Reed Rd.) added to I/O program in late 2005. Dave Hodges indicated that wells were added to program. Initial complaint was # 1247 received 10/4/05. Mark indicated in his call that this & the other 2 wells were getting worse. Well was a gas well. South well has 8 casing. The well was plumbed up during 10/13/05 and 3/15/06 inspections and are still plumbed up. Well has 2 production line that is burined. Valves are closed. Small storage resevior & small regulator attached to well. Since last inspection 2 gopher holes have been dug on well. Soybeans growing north of the well. No oil seen on ground. Photos taken. On site to check well & update priorty matrix. |
1130339526 | 2005-10-13 | Orphan | No Violations | -On site after complaint # 1247 inspection for I/O by landowner Richard Bruns across Reed Road. Met w/ Mark Dahlinghaus, one of the trustees for the Donald J. Dahlinghaus Trust property at 10459 Reed Road. Three I/O gas wells on site. Mark indicated the 3 wells on his property was tied in with the Bruns well on west side of Reed Road. Mark indicated the he believed the wells were drilled by a former neighbor Linus Poeppelman (now deceased) about 50 years ago. Well record for the # 3 well (20009) shows the 3 wells drilled by Poeppelman Gas Co. Mark indicated that he thought a lease had been drawn up in the past but a courthouse search later in the day at the Darke County Recorders office listed no oil/gas lease(s) ever being recorded. Well # 1, the southernmost well, has 8 casing. The well appears plumbed up. Well has 2 production line that is buried. Valves closed. Small storage resevior & small regulator. Well located in crop field. No obstructions. 3 photos taken. Mr. Dahlinghaus left with Right-of-Entry forms to complete and submit. |
1142875726 | 2006-03-15 | Orphan | No Violations | -On site per Dave Hodges request to take additional pictures. Several of the three wells on the Dahlinghaus property were photographed with the digital camera on the movie setting thus leaving a grainy view of the wells. Several additional photos of well # 1 also taken and e-mailed to Jeff Fry and Dave Hodges on 3/16/06. Dave Hodges indicated that this well and the other 2 wells on the site have been added to the I/O well program. |
1210707639 | 2017-08-30 | Plug / Plug Back | No Violations | It was determined to combine the bottom of the surface and the top of the surface plugs into one plug. Tubing was placed at a depth of approximately 513 feet deep and 55 sacks of Class a cement was installed. Cement was not observed at surface. |
1245310940 | 2012-09-17 | Orphan | No Violations | An inspection revealed that the well consisted of 8casing above ground with an 8-2 swage. The swage contained material resembling cement around the outside to serve as a seal. A flange was installed at the top with steel routed to a separator located next to the well. From this location, the steel production line was then routed underground. A concrete block resembling the shape of a rectangle approximately 10 x 5 surrounded the immediate well location. There are no records for this well. Therefore, a review of the surrounding area revealed that the wells drilled in this area were to the Trenton formation approx 1200-1225. The well is situated on a property designated as agricultural field (bean field) with no overhead obstructions. Note: Field tile exists on the property. A slight gas odor was smelled during the inspection. A possible entrance location could be the driveway leading to the residence (farm house). If not utilized, a culvert may need to be installed for entrance off road. |
1260007475 | 2017-08-31 | Plug / Plug Back | No Violations | Measured top of cement on this well and it was determined to be at a depth of approximately 66 feet. Six sacks of Class A Cement was installed and cement was observed at surface. |
1554933603 | 2017-11-21 | Final Restoration | No Violations | Final restoration is not approved. Landowner waiver will be necessary because of buried steel line. Landowner does not want line to be removed because of potential to damage field tile. |

1601750111 | 2017-08-24 | Plug / Plug Back | No Violations | Orphan well contractor ran 1.5 inch tubing to bottom of this well in order to place Class A cement for bottom plug on well. Seventy sacks of Class A cement was installed through tubing . |
1853512487 | 2012-10-24 | Orphan | No Violations | A follow-up inspection was conducted with Jeff Fry. During the inspection, it was noted that the well contained a molded 8 concrete wellhead. Above this mold, an 8 bolted flange contained 2 gas fittings, and a gate valve before the connection into nearby separator and regulator. The overall form of this concrete to area just below the flange resembled the shape of a swage. Additional concrete was also poured around the immediate well location. The concrete walls were flush with the ground and measured 82 x 44. A gas odor was smelled and verified with the gas detector. The readings were as follows: O2= 20.9, CH4-IR= 0 - 0.05, and LEL= 1. |
2125916962 | 2017-08-21 | Plug / Plug Back | No Violations | Appalachian well service was on site to log wells to determine plugging requirements. Total depth of well was determined to be 1195 feet deep and bottom of the 4.5 inch surface casing was determined to be at a depth of 340 feet deep. |
