Conductor casing had not been cut off, IDed cap placed, and area filled in since. After leaving site on 11/9/11, Bob Huwer did dig out around conductor casing, cut pipe off 30 below ground, and placed cap. When at the site on 12/13/11, photographed to cap labeled DNR 44 and watched as Bob Huwer filled back in the excavated pit. Photo taken of the broken off cement from top of well down to 30. Photos taken of the reclaimed area. Reclaimed area in farm field and will be planted in spring 2012. 100 bbl tank still present by well on 12/13/11. Talked to Bob Huwer on 12/20/11 & he brought in vacuum truck to pump out oil on 12/18/11 and then removed the tank on 12/19/11. Final restoration of the well to be given. Complaint # 3185 updated on 12/14/11 & notified Will Ziegler to close out the complaint. Huwer requested copy of the inspection report & plugging report & will send to him on 12/21/11. |

Land was up for auction & she is in a nursing home. On site w/ R. Stonerock & Ben Harpster (NW Ohio O/G inspector) to check complaint. This well is located SE of the farmhouse. 100 bbl tank about 20 NE of the well. Well has 8+ conductor surface casing w/ swedge reduced to 2. Rods, homemade pumping unit w/ electric motor no longer hooked up. Cement pad approx. 6 by 2.5 poured for base of unit & also poured outside of the conductor. Title search later indicated property is Mary Louise Kelchs property & Probate Court search indicates the wells on the property are owned & the responsibility of M.L. Kelch after settlement agreement. No lease assignment or bill of sale in recorders office. Rob Stonerock talked to Deputy Chief Tom Tugend and Tom contacted atty Eric Brand of Goubeaux & Goubeaux law firm (937-548-2211). E-mail from Rob Stonerock indicated Tom & Eric discussed an escrow agreement. Tom turned the matter over to DMRM atty Sandy Ramos. Auction for property ends 7/8/11. Well is not labeled, incapable of production, & not registered. |

I called Bob Huwer on 12/20/11 and he indicated that he had moved the tank from the site on 12/19/11. Final restoration was granted at that time. Drove by site to make sure tank was removed on way to nearby I/O wells. Since 12/13/11 inspection, old farmhouse on site torn down. Well owner already granted final restoration toward the end of 2011. |

(cement is from Lafarge according to Bob). Bob planning on 11/8/11 to pump any oil/fluids out of the well, then pull the 6 5/8 surface casing. Earlier in the day, Bob & Aaron Huwer of Huwer Well Drilling had pumped out oil and fluids in the well. Bottom plug of 300 feet of Class A cement set on 11/7/11. Once well pumped, operator filled the hole with water as a spacer down to 450 feet below the surface. Huwer began pulling the 6 5/8ths surface casing. 15 joints recovered at somewhere over 300 feet (approx. 320 feet). Operator then contacted St. Henry Tile Co., Inc. and ordered 6 cubic yards of 9 sack grout. Grout was sent down the outside of the tubing but inside the conductor pipe with water and fluids circulating up the tubing to the tank truck. Grout circulated to the surface. Huwer to let the grout settle before cutting the conductor pipe 30 below the surface, attaching the ODNR labeled cap, and covering over the hole. 100 bbl tank to be removed. Rain on 11/9/11 canceled final restoration activity at the site until the week of 11/13/11. |
