0403939377 2011-06-20 Production Wells Violations Noted -On site per anonymous complaint # 3185 rec. 6/13/11 by Rob Stonerock re: 3 wells on Mary Louise Kelch propety. Land is up for auction & she is in a nursing home. On site w/ R. Stonerock & Ben Harpster (NW Ohio O/G inspector) to check complaint. This well is located east of the farmhouse. 8+ conduction, 6 surface casing w/ swedge reduced to 2. Rods, homemade pumping unit w/ electric motor no longer hooked up. Gas separator & prod lines still hooked up. Six 55 gallon steel drums next to well. Well not labeled, not capable of production, not registered w/ state. Title search later indicted property is Mary Louise Kelchs property. Probate Court search reveals the wells on site are owned & the responsibilty of M.L. Kelch per settlement agreement w/ other partners. No lease assignment or bill of sale found in Recorders office. Rob Stonerock talked to Deputy Chief Tom Tugend and Tom contacted atty Eric Brand of Goubeaux & Goubeaux law firm (937-54802211). E-mail from Rob Stonerock indicated that Tom & Eric discussed an escrow agreement. Tom turned the matter over to DMRM atty Sandy Ramos. Auction for property ends 7/8/11. Well is not labeled, incapable of production, and not registered.
1123639531 2014-03-11 Administrative Inspection No Violations Complaint database follow-up. This well was plugged 12-9-11. This satisfies the complaint.
1324396060 2011-12-09 Preliminary Restoration No Violations -On site at Kelch # 2 (6-0045) for bottom plug being set. Between the 6/20/11 initial insp & 11/7/11 insp for bottom plug being set at Kelch # 1 (6-0044), Bob Huwer has removed the barrels, swedge, pump jack, rods, and 2 production casing from the well. Vegetation that grew up around the wellhead cleared. Well depth is approximately 1215 ft. according to Bob Huwer. Surface casing in well is 6 5/8th. The bottom plug, 6 cubic yds of Class A cement from St. Henry Tile Co., Inc. pumped down to well to create around a 300-400 foot bottom plug. After bottom plug set, Huwer planning to pump out fluids, recover whatever surface casing he can before top plug set. Photos taken. Cement ticket received from St. Henry Tile driver.
1324416555 2011-12-13 Final Restoration No Violations -On site for Kelch # 2 (6-0045) for top plug being set. The bottom plug was set on 12/9/11. After 12/9/11 bottom plug set, fluids in well pumped. About 310 ft of 6 5/8ths inch diameter surface casing removed & fresh water spacer pumped. 5 cubic yards of 9-sack grout pumped down the annulus of the well and water & fluid circulated to the surface to create a 450 foot top plug. Fluids pumped to shallow pit beside the well. Later in the day, the well dug out around, the conductor casing cut off. The cement in the cut off casing, which had not set up at surface was noted in the pit. The IDed well cap placed over well. The well & shallow pit filled back in and final restoration done.
