1309433286 2011-06-20 Production Wells Violations Noted -On site per anonymous complaint # 3185 rec. 6/13/11 by Rob Stonerock re: 3 wells on Mary Louise Kelch property. Land was up for auction & she is in a nursing home. On site w/ R. Stonerock & Ben Harpster (NW Ohio O/G inspector) to check complaint. This well is located N. of the farmhouse. Well has 8+ conductor, 6+ surface casing w/ casing head. Tubing & rods not present. Well covered by blue plastic 55 gallon drum. Title search later indicated property is Mary Louis Kelchs property & Probate Court search indictes the wells on the property are owned & the responsibility of M.L. Kelch after settlement agreement. No lease assignment or bill of sale in Recorders office. Rob Stonerock talked to Deputy Chief Tom Tugend and Tom contacted atty Eric Brand of Goubeaux & Goubeaux law firm (937-548-2211). E-mail from Rob Stonerock indicated that Tom & Eric discussed an escrow agreement. Tom turned the matter over to DMRM atty Sandy Ramos. Auction for property ends 7/8/11. Well is not labeled, incapable or production, and not registered.
1322492680 2011-11-21 Preliminary Restoration No Violations -On site at Kelch well # 3 (6-0047) for the bottom plug. On site with Ben Harpster and Tom Benko. Bob and Aaron Huwer with Huwer Well Drilling. Since 6/20/11 inspection, Huwer has removed above ground hardware. No production casing in well according to Bob. Well depth at 1216 feet with 6 diameter pipe. 5.25 cubic yards of Class A cement pumped to set bottom plug. Plug about 400 feet thick but will be measured with wire line before top plug set. St. Henry Tile Co. provided cement (Lafarge is cement mfg). Once bottom plug set (and weather depending), operator to pump out the fluids/oil in well, pull surface casing, then plug 100 feet below to surface casing and to the top of the hole. Corn harvested from around well site since 11/18/11 inspection on 34-037-6-0044-00-00.
1324388264 2011-12-08 Preliminary Restoration No Violations -On site at Kelch well # 3 (6-0047) for top plug. Bottom plug of 5.25 cu yds of Class A cement, about 400 thick, set on 11/21/11. The above ground hardware removed between the 6/20/11 & 11/21/11 inspection. No production casing or rods in well according to Huwer. Well depth at 1216 feet w/ 6 diameter casing. Between the 11/21/11 & 12/8/11 inspections, the fluids & oil in the well was pumped out. There was around 300 feet of 6 surface casing recovered from the hole according to Bob Huwer. 5 cu yds of 9 sack grout from St. Henry Tile Co., Inc. used for the approxmetly 450 feet top plug. Grout sent down outside tremie pipe & circulated to the top thru the pipe. Fluid in hole run to the field rather than tanker truck due to vacuum pump froze up. Photos taken of the plugging. Once cement set up, area around well to be dug to 30, conductor casing cut off, well ID cap placed & area around well filled back in.
1554370423 2011-12-13 Final Restoration No Violations -On site for final restoration inspection along w/ top plug for well 34-037-6-0045-00-00 (Kelch # 2). Top plug for well 6-0047 was set 11/21/11. Top plug set 12/8/11. When arrive at site on 12/13/11, Bob Huwer indicated that farmer said he wanted to chisel the field so after well 6-0045 bottom plug set on 12/9/11 & after I left the site on 12/9/11, the area around the well dug out, top plug & conductor casing cut off 30 below ground, well cap w/ ID # DNR 47 placed, and area around well filled in. Field was not chiseled. Photos taken on 12/13/11 of the final restoration. Cut off top plug of cement still on site to view before being removed on 12/13/11.
