0080267662 2018-10-18 Plug / Plug Back No Violations Columbia Gas has perforated the 7 inch casing at 1723 and 1400 feet. They set a cement retainer at 1715 feet. Kilbarger placed the drill pipe into the cement retainer and Petroset set 43 sacks of cement thru the retainer. Kilbarger pulled out of the retainer up 340 feet and set 66 sacks of cement. This was done to place a plug at the big lime.
0255422250 2018-10-09 Plug / Plug Back No Violations Petroset Cementing on site at 10:30 AM. They circulated the hole with 100 barrels of water. They ran 50 sacks of class A cement displaced with 6 barrels of water. The cement was mixed with 2% calcium and 5% salt. This plug will be tagged on 10.10.18 to determine where the top of the plug is. The well was drilled in 1944 and would have had a cavity at the bottom of the well bore. There is no way to know the size of the cavity so tagging the plug will confirm the height of the plug.

The bottom of surface plug took 80 sacks of cement and the surface plug took 120 sacks of cement. I did not witness the plugs being set.
0645097081 2018-10-16 Plug / Plug Back No Violations My inspection revealed Columbia Gas tagged the Newberg plug at 1945 feet. They ran a casing cutter inside the 7 inch casing and cut the 7 inch casing at 1723 feet. I will check on there progress on 10.17.18 to determine if they were able to pull the 7 inch casing from 1723 feet.
0676229033 2018-10-02 Plug / Plug Back No Violations I arrived at 1:10 PM to meet with Columbia Gas representative Mr. Dan Talbert. Mr. Talbert had a caliber log ran today and found the well still has 168 feet of 5.5 inch diameter casing in the bottom of the well bore. Columbia Gas is deciding if they want to mill out the top of the 5.5 inch diameter casing so a spear can be used to retrieve the remainder of the 5.5 inch diameter casing.

1077741709 2018-10-22 Plug / Plug Back No Violations Kilbarger removed 270 of 8 inch diameter casing. The was 675 feet of 8 inch casing ran when the well was drilled. They attempted to run tubing down to set the bottom of surface plug but ran into a obstruction at 271 feet. It is believed the hole caved in when the 8 inch casing was removed. They will use a power swivel to drill out the obstruction. Petroset cementing on site at 2:20 PM. I did not witness this plug being set.
1282772683 2018-10-15 Plug / Plug Back No Violations Halliburton cementing services on site at 1200 hours. The plan was to sting tubing through a cement retainer set at 2150 feet. They placed the tubing through the cement retainer and pumped 21 sacks of cement. This plug was to place a plug covering the newberg formation. Columbia Gas had the 7 inch diameter casing perforated at 2160 feet. The 21 sacks of cement was designed to exit the perforations and cover the newberg formation. 24 sacks of cement was placed on top of the cement retainer. Columbia Gas will have the 7 inch diameter casing shot off on 10.16.18.

1912215491 2018-10-01 Plug / Plug Back No Violations Columbia Gas hired Kilbarger to plug this well. Kilbarger pulled 2216 feet of 5.5 inch diameter casing. On the end of the 5.5 inch diameter casing is what looks like a packer. Columbia Gas is not certain all of the 5.5 inch diameter casing was removed from the hole and will run a caliber log on 10.2.18 to determine if any more 5.5 inch diameter casing remains in the well bore.
2025361102 2018-09-19 Plug / Plug Back No Violations I met with Columbia Gas representative Mr. Dan Talbert regarding plugging this well. We went over the minimum requirements Mr. Talbert showed me Columbia Gas plugging plan it met all the requirements to plug this well. Kilbarger on site moving in plugging equipment in preparation of plugging the well on 9.24.18.
2055281652 2018-10-11 Plug / Plug Back No Violations I arrived at 1:40 PM and met with Columbia Gas well technician Mr. Dan Talbert. He has ordered 30 sacks of cement to be placed on top of the bottom hole plug. The bottom hole plug that was set did not meet the minimum requirement for a bottom hole plug. Haliburton set this plug today. I did not witness this plug.
