0234276468 2010-08-27 Orphan No Violations This morning showed that the well is leaking gas to the surface between the 7inch casing and the 10 inch casing. It is not leaking outside the 10 inch casing. It is not leaking inside the 7 inch casing. A cement bond log was ordered and performed by Appalachian Well Services. There is approximately 70 feet of cement left inside the bottom of the 7 inch casing. A copy of the CBL will be avaliable in the well file. Berry Bowers, Bill Mitten, and Mike McCormac were on location. Weather: 84F, wind NW at 3mph, sunny, dry. Time Duration: 4.25hr on location, 2.5hr drive time.
0374997008 2010-08-31 Plug / Plug Back No Violations A total depth of 1001 feet was made today and the top of the plug was tagged at this depth. There has been no new gas encountered since 380 feet. Bill Mittin and Berry Bowers was on location.
0633222062 2010-08-25 Orphan No Violations Jeff Fry covered the morning activities. The drill pipe was unplugged and tripped back into the hole. A drilling mud slurry was mixed up and drilling continued. Drilling was faster and a TD of 302 feet was reached by 7pm. Mike McCormac and Scott Parker were on location for part of the day. The drill stem and bit plugged up with sand and gravel to the point where the rig crew had to shut down and trip out of the hole. Bill Mittin ordered drilling mud so that tomorrow they can drill out borehole on mud to prevent cave-ins. Berry Bowers was on location. Jeff Fry and Scott Parker were on location part of the day. Weather: 82F, wind E at 5mph, partly cloudy, dry. Time Duration: 12hr on location, 2hr drive time.
0745739471 2010-08-16 Plug / Plug Back No Violations I met with Bill Mitten and Berry Bowers on location. Pat Romine with VT Services was also on location as the welding contractor. The compression end cap that was placed on the wellhead by Columbia Gas was removed. When removed, it revealed that the well bore has been filled to the surface with gravel and screenings. A ten inch threaded nipple was then welded onto the wellhead by Pat Romine. There was no change in the well status as it continues to vent gas to the atmosphere at a very small rate.
0804890970 2010-06-01 Orphan No Violations I inspected this well location today at the request of Tom Tugend. The well status has not changed from the previous inspection. The temporary stack pipe is still open and venting a small amount of gas to the atmosphere. There is a new home that has been built on the lot approximately 75 feet from the wellhead.
1260801572 2009-12-11 Orphan No Violations I received a phone call today from Brenda VanCleave with the city of Pickerington concerning this well. She indicated that this casing was uncovered during the construction process of preparing to build a new home in this neighborhood. It was uncovered approximately two weeks ago and was emitting natural gas. She also indicated that Columbia Gas was contacted and responded by placing a steel compression end cap on the ten inch casing and shut the well in. I inspected the well location today. There is a steel wellhead secured to the well casing and the well is shut in. I was unable to attach a pressure gauge to this wellhead. The gas meter detected no gas. The well head is approximately 3 feet from the edge of the road and across the street from 861 Avalon Drive, Pickerington. I took photographs of well and surrounding area. RBDMS shows that this well was drilled in April of 1964 and plugged in March of 1967 with fireclay. I will refer this well to Mike McCormac with the Division. Also, I will advise Brenda VanCleave and Columbia Gas to open the wellhead and vent it to the atmosphere. Well records show 40 feet of 10 inch casing. Todays weather was below freezing and very windy.
1260887148 2009-12-14 Administrative Inspection No Violations I called Brenda Vancleave at 614-207-8350 today and advised her to contact Columbia Gas and request them to remove wellhead dead end cap. Columbia Gas had placed the cap on the wellhead approximately 2 weeks ago. RBDMS shows 40 feet of 10 inch casing in the well. I also advised her that a vent stack pipe, extending at least 9 feet above grade needed to be placed over the well head. I requested that I be notified at least 3 hours before the wellhead is removed so that I could be on location to monitor well activity.

1282580719 2010-08-23 Plug / Plug Back No Violations Berry Bowers and Bill Mittin were on location with crew plugging well. From 7am until 12:30pm two runs were made with homemade fishing tool built from 8 inch casing. Tool is approximately 4.5 feet long with teeth cut on the end. The fishing tool was filled with granite field stones and fireclay on the first run. The stone sizes varied between golf ball and football size stones. On the second run the tool only brought back clay. This morning approximately 5 foot of hole has been made. The rig crew switched over to drill with 8 3/4 inch rotary bit. By 7pm a TD of approximately 110 feet has been made. Scott Parker from the city of Pickerington also stopped by location for a short time. Weather: 80F, wind NE at 8mph, overcast morning, sunny afternoon, dry. Time Duration: 11.75hr on location, 2.5hr drive time.
1282651352 2010-08-19 Plug / Plug Back No Violations Berry Bowers and Bill Mitten with plugging crew were on location. Scott Parker from the City of Pickerington also stopped by for a short while. Drilling was very slow today while attempting to clean out the surface hole. Rig is drilling with a cable tool power swivel unit, using a 8 3/4 inch bit. By the end of the day approximately 50 of hole was made. Chunks of wood and gravel were drilled out of the well. A fishing tool will be constructed this evening out of 8 inch pipe in an attempt to fish out obstruction. Weather: 91F, wind NW at 3mph, sunny, dry. Time Duration: 12hr on location, 2hr drive time.
1285164043 2010-08-30 Plug / Plug Back No Violations The first thing this morning the well showed a small amount of gas present on the inside of the 7 inch casing. Encountered gas at 380 feet that blew for a couple of minutes into steel tank. A total depth of 470 feet was reached today in the clean out process. Bill Mitten and Berry Bowers were on location.

1369339315 2010-09-07 Plug / Plug Back No Violations Bill Mitten and Berry Bowers were on location. I was not on location. They used a digital gauge that showed the wellhead had zero pressure on it after sitting shut in over the weekend. A plate was placed on the wellhead and buried below grade.
1550598491 2010-04-16 Administrative Inspection No Violations Jeff Fry and I met with plugging contractors, City representatives, and others at this well location in order to discuss the details of this emergency I&O plug. Mike McCormac and Tom Tugend from the Division were also present. Berry Bowers and Bill Mitten were the contractors on location. Scott Parker and Brenda VanCleave were with the city.
1577625502 2010-09-02 Plug / Plug Back No Violations This morning showed that there was no change in the small gas leak on the backside of the 7 inch casing. There is no gas leak on the inside of the casing. A ripping knife tool was used to rip the pipe at 120 feet and 90 feet. A telegraph was detected at both locations. Several pieces of steel were recovered indicating that there were holes knocked into the sides of the pipe at both locations. The gas stopped leaking on the backside of the 7 inch and began leaking on the inside of the pipe. Bill Mitten and Berry Bowers were on location.
1617073465 2010-09-03 Plug / Plug Back No Violations This morning showed that there was still a small gas leak inside of the seven inch casing and none on the backside. The pipe was ripped at 120, 90, and 60 feet. The pipe parted at one of those locations. The casing was moved vertically 12 inches at the surface. The wellhead was pumped on with fluid and the well circulated. The well bore was bailed dry from top of 300 foot plug to surface. A cement plug was set this afternoon using 4 yards of 9 sack grout. After cement was placed into well the mud pumped pressured up on the wellhead to 700 pounds in order to squeeze cement. The well was filled from 300 foot to surface with cement.
1657854457 2010-08-18 Plug / Plug Back No Violations Berry Bowers and Bill Mitten with plugging crew were on location today and began the process of drilling out this I&O well in order to properly plug it. Scott Parker with the City of Pickerington, Jeff Fry and Mike McCormac with the Division were all on location during parts of the day. Drilling was very slow and by the end of the day a TD of approximately 20 feet of hole was made. The drilling crew is working with a Walker-Neer cable tool rig and power swivel unit. Fresh water is being circulated into steel tank set in the street. There is also a stand by frac tank on location. There is a water vac truck also on location. The fresh water source is from a nearby city fire hydrant. There is a liner set up on location under equipment and a straw bale dike set up on the downhill side of location. Weather: 90F, wind W at 3mph, sunny, dry. Time Duration: 10hr on location, 2hr drive time.
1674315462 2010-08-17 Plug / Plug Back No Violations I inspected the well location this afternoon with Jeff Fry. Berry Bowers and Bill Mitten are in the process of moving equipment to this location in order to plug this I&O well. There was nobody on location. A frac tank and backhoe have been dropped off. Tomorrow they will begin the clean out process in order to plug well. Time Duration: .5hr on location, 2hr drive time.
1745395441 2010-08-12 Orphan No Violations Brenda VanCleave from the City of Pickerington called today. She indicated that a resident from the neighborhood close to 861 Avalon Drive notified of the presence of a gas odor. Fire trucks were on location as was Brenda VanCleave. There was no change in the well status as it continues to leak a very small amount of gas to the atmosphere. The plugging of this well is scheduled for next week.
1950025302 2010-02-02 Orphan No Violations Jeff Fry and I inspected this well location today. This well has a dead end cap well head on the drive pipe casing and is open to the atmosphere. It is venting a small amount of gas. Jeff and I filled out the I&O report and took photos of this well.
1956999428 2010-09-04 Plug / Plug Back No Violations This morning showed that the cement remained at surface level. There was evidence of a very small amount of gas bubbling at the surface inside of the 7 inch casing. The gas was not enough to maintain a flame, as Berry Bowers tried to light it. It was determined to weld a casing plate on the 10 inch with a 2 inch nipple on it. This would allow us to close the wellhead in with a valve and pressure gauge to see if there was sufficient gas to build any pressure.
1973589609 2010-08-26 Orphan No Violations A total depth of 327 feet was reached by 11am. 312 feet of 7 inch surface casing was run into the well. Cemenco came at approximately 2pm and completed a surface cement job. Details of the surface cement job can be seen on the surface cement ticket. Bill Mitten, Berry Bowers, Jeff Fry, Brenda VanCleave, Scott Parker were all on location some today. Weather: 81F, wind NW at 5mph, sunny, dry. Time Duration: 10hr on location, 2hr drive time.
2111857359 2010-09-01 Plug / Plug Back No Violations Since no new gas has been encountered since 380 feet and the top of the plug was tagged at 1001 feet, it was determined to plug the well from this depth out. A 196 sack cement plug was set today by Formation Cementing. This cement plug was set from approximately 950 feet to 300 feet. I was not on location during the cement job, but Mike McCormac with the Division was. Bill Mitten and Berry Bowers were on location as well.
