0082861133 2016-05-05 Preliminary Restoration No Violations The purpose of this inspection is a status check on a preliminary restoration. During the time of my inspection I witnessed that the well is equipped with 4.5 inch production casing, 2 inch tubing, and rods. The 2 inch tubing was shut in and the 4.5 inch casing had an opened line hooked up leading to a drip. 2 inch tubing, rods, and hoses were lying around the location of the well. There was a pumping unit off to the side of the well that was not hooked up. There was no identification present during inspection. Spotty vegetation has grown back and the lease road was in good shape. Preliminary restoration can not be passed at this point until the 2 inch tubing, rods, and hoses are removed from the location.

I contacted ODNR division of oil and gas Permitting manager Mr. Steve Opritza concerning the status of the well. I signed off this well for preliminary restoration in a previous inspection.

The tank battery consists of 2-210 barrel tanks. This well shares the tank battery with 3404520283 and 3404521311. The tank battery and the well had legible identification. The well meets the requirements for preliminary restoration.
