| 0989751597 | 2007-03-13 | Production Wells | No Violations | The leak has been temporarily repaired by Pete Jeffers Orwig Pumper. I had a meeting with AEP and the logger to review what had happened. I advised AEP to have the logging operation leave a buffer zone around the wells in the work area. I called the Gallia Emergency Management office and advised them on the status of the well. |
| 1199117548 | 2007-11-28 | Plug / Plug Back | No Violations | On 11-21-07 I was at our meeting in Cambridge and was unable to witness the plugging of well #246 this is one of the AEP wells being plugged for their land fill. The bottom hole plug was set on 11-21-07. This is a Berea well. TD 1801. 300 7 in hole. 1800 4.5 in the hole. I did not receive any phone calls telling me that they had problems. The information from here on is second hand or taken from Timcos invoice and Formations invoice, I will try to put it in a time line.
Rig TD top of 1st cement plug at 1475. This is where it should be. Had calculated 1420 so it was close. Timco (wire line) arrived. They could only get to 1228 so with out calling me they decided to shoot it at 1190. This is above where I told them to start the Big Injun Plug at 1250 to 850. They shot one time. Rig tried to pull free. Could not pull free. They were able to circulate the hole to the surface. They decided to bullhead the 2nd plug at this time. Formation Bullheaded the 2nd plug. Timco tried to shoot again at 1161. Rig could not pull free. Circulated the hole to the surface. Formation Bullheaded 3rd plug. Timco tried to shoot at 637 Rig could not pull free. Circulated hole to the surface. Formation Bullheaded 4th plug. Timco tried to shoot at 300 Rig could not pull free. Circulated to surface. Formation Bullheaded 5th plug. 52 of 4.5 was all that was recovered. They unscrewed it. The Cement job including setup and cleanup was from 12:40 pm to 4:00 pm. I do not feel that 3 hours and 20 minutes was enough time to make a good faith effort in recovering the casing and plugging the well correctly. I believe that a second effort should have been made to recover the casing at all areas that were attempted. One try and giving up does not in my mind make a good faith effort. I arrived on site at 4:00 pm and passed Formation on the road. I was first lead to believe that 300 of 4.5 had been recovered. The rig was all ready moving to the next location. I went to the next loca |
| 1211241820 | 2007-03-12 | Production Wells | No Violations | Well has been severely damaged by logging activity. Well casing and tubing has been bent and leaking badly. Other damage was done to the pump jack and to production piping. I informed AEP of the problem. I called the Gallia county sheriff and the Gallia Emergency Management of the leak and the location. I called Anita Orwig in Florida, she gave me the name of her tender Woody Harris. I called Woody Harris and he gave me the name of the pumper Pete Jeffers. They informed me that they would be on site in the morning. I also called the logger Art Saylor and informed him of the situation and warned him not to take any equipment near the leak for fear of ignition. The loggers are using the area around the well to stock pile logs for processing. This makes a lot of fuel around the leaking well. |

| 1610579627 | 2008-04-03 | Final Restoration | No Violations | Ohio Valley Electric Corp. is constructing a landfill for FGD generated from the Kyger Creek Power Plant. The well site is located within the landfill project area and excavation to the location is underway. At 4:01pm on 12-4-08. I talked to Missy. A single page was faxed to ODNR-DMRM Jackson Office at 5:57pm on 12-4-08. Single page was for date 11-28-07 invoice # 11630. They did not send a copy of the job log for 11-21-07. |
