Correction to Well 2006-02-28 INSPECTED BY LINDA SLATER ON FEBRUARY 21, 2006. THE WELL IS QUITE A MESS. RBDMS HAD THIS WELL AS NOT DRILLED BUT THERE ARE SURFACE CASING AND PRODUCTION CASING REPORTS AS WELL AS A PRELIMIINARY RESTORATION REPORT.

1056941994 2006-02-22 Production Wells No Violations Operated by Luther and Grace Lemley. Provides gas to three family member homes and to one rental home. Grandaughter pumped well when line valve to tank shut off. Oil and brine came up through the stuffing box rubbers and sprayed over site, jack and well head. Copious quantities of brine and oil ran into the mud pit that was never closed. Line leak near separator causing ground contamination. Well showed as not drilled in RBMS. Landowner said Nick Ross drilled the well in 1979.
1410093472 2015-06-04 Production Wells No Violations Follow up to -1797387381. The leaking lines have been replaced and much of the contaminated soils have been removed and that area has been seeded and mulched. There is still some contaminated soil around the area of the separator.
1438273709 2015-07-30 Production Wells No Violations Follow up to -1797387381. The contaminted soils have been removed and the area affected was sseded and mulched. A dike was constructed around the swab tank and seperator that appeared sufficient to contain any leaks from the swab tank in the future. The leaking equipment was replaced. I will be mailing a form 7/7a to Ms. Nancy Lemly to make sure the well is transferred into her name properly.
1797387381 2015-04-24 Production Wells Violations Noted I was notified of a small leak by a producer in the area of this well. Upon inspection I found a 1100 gallon tank near the separator to have over filled with roughly 5-10 gallons of luid having run down the side and contaminated a 15 x 30 foot area. Steel lines from the sperator had been replaced with PVC. There was no dike in place around the tank or separator. The flow had been stopped from reaching a small field drain with absorbent pads and shovels by the producer who notified me of the spill. I saw no evidence of any contamination in the drain at the time of inspection. The vavles on the well itself were open and the well was supplying housegas to the landowner. There was no identification at the well head or any of the tanks. There was also an empty 100 bbl steel tank on site that was not connected to anything, there was a 10 x 5 foot area near the base of this tank that looked as if there had been contamination in the past. RBDMS ownership records show WJ Lydic to be the owner but I spoke with Cathryne Smith in Columbus and she stated we showed Nancy Lemley, the landowner, as the well owner. Any leaky or faulty equipment shall be repaired or replaced. All contaminated materials shall be removed and disposed of properly. Any effected areas shall has soil replaced and area shall has vegetative cover reestablished. Storage tanks shall be placed inside a dike of suffient size to prevent contaminated materials from leaving the site in the event of another release of fluids. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number
